The On Call Registered Nurse will be responsible for responding to urgent patient and family needs, managing symptom control, and providing support during end-of-life care. This role requires a timely response to calls, effective clinical assessment, and the ability to work independently.</p><p><br></p><p><br></p><p><strong>Job Qualifications:</strong></p><p><strong>Education:</strong> Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.</p><p><strong>Licensure:</strong> Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.</p><p><strong>Experience:</strong> Experience as a registered nurse in a clinical care setting. Hospice experience preferred.</p><p><strong>Skills:</strong> Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write, and comprehend English.</p><p><strong>Transportation:</strong> Reliable transportation and valid auto liability insurance.</p><p><br></p><p><strong>Environmental and Working Conditions:</strong></p><p>Works in patients’ homes and facilities in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.</p><p><br></p><p><strong>Essential Functions:</strong></p><ul><li>Provide on-call hospice nursing care in response to patient or family needs during assigned shift.</li><li>Perform patient assessments, manage symptoms, and administer medications as needed.</li><li>Collaborate with the interdisciplinary team (physicians, social workers, chaplains, hospice aides, etc.) to ensure comprehensive care.</li><li>Consults with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.</li><li>Maintain accurate and timely documentation of care provided using EMR.</li><li>Communicate changes in patient condition to the care team and recommend adjustments to the care plan.</li><li>Provide emotional support and guidance to patients, families, and caregivers.</li><li>Ensure that all interventions adhere to hospice philosophy, polices, and regulatory guidelines.</li><li>Maintain timely response to on-call duties, ensuring patient and family needs are met in a timely manner.</li><li>Triage duties may be required for some employees in this role, depending on specific assignments and departmental needs.</li><li>Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.</li><li>Informs physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care.</li><li>Demonstrates commitment and professional growth and competency by participating in the Hospice sponsored in-service programs and maintaining or improving competency.</li><li>Promotes the Hospice’s philosophy and administrative policies.</li><li>Provides effective communication to patients, family, team members, and other health care professionals.</li><li>Perform emergency admissions of new hospice patients as needed.</li></ul><p><br></p><p><strong>Benefits: </strong>Referral Program and 401(k)</p>",
