The Clinical Coordinator is a registered nurse who has clinical expertise to the Subspecialty providers in providing quality medical care to patients.. The Coordinator is responsible for providing professional nursing care and support to patients/families with a variety of subspecialty needs. This position will be a liaison between the family and the subspecialty team to assist in care coordination and disease specific education for patients, families, and staff.</p>\n<h2>Responsibilities</h2>\n<p>A day in the life of a <strong>Clinical Coordinator</strong> at Hackensack Meridian Health includes:</p>\n<ul>\n <li>Maintains a communication system which ensures comprehensive and timely transmission of information to providers and other members of the multidisciplinary team. Applies evidence-based practice to make clinical decisions and establish standards of clinical specialty. </li>\n <li>Interacts with patients /family during provider office visits and via phone, assessing patient needs. Collaborates with providers and assists with developing evidence-based solutions to meet desired measurable outcomes. </li>\n <li>Documents all interventions, assessments, plans of care, interactions, care provided and findings in EMR. </li>\n <li>In collaboration with the multidisciplinary team participates in the assessment, planning, implementation and evaluation of care for the patient population. Provides patient care including nursing assessments. </li>\n <li>Provides additional support to the subspecialist to enable the provider to focus on the highest level of care. Facilitates diagnostic testing,prescription refills and other patient instructions requiring provider signature. Acts as a liaison with other departments to clarify questions related to care of the patient. </li>\n <li>Serves as a clinical resource person to the staff, patients/families, and other team members access the continuum to evaluate complex patients and identify appropriate desired outcomes and interventions.</li>\n <li>Serves as a resource for clinical staff, developing and delivering education and operation guidelines. </li>\n <li>Participates in the development of evidence based guidelines/protocols that are utilized in planning patient care and monitoring achievement of patient outcomes.</li>\n <li>Participates in continuous process improvement efforts to improve patient outcomes, assures appropriate utilization of resources and increases patient satisfaction. </li>\n <li>Responsible for subspecialty clinic oversight of supplies, coordinating clinics with providers, scheduling, and providing clinical direction to the assistant care coordinator. </li>\n <li>Maintain awareness of standards and regulatory agency compliance. </li>\n <li>Other duties and/or projects as assigned. </li>\n <li>Adheres to HMH Organizational competencies and standards of behavior.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education, Knowledge, Skills and Abilities Required</strong>:</p>\n<ul>\n <li>Bachelor's of Science in Nursing </li>\n <li>Minimum of 3-5 years acute/primary care for specialty and population specific nursing experience </li>\n <li>Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms. </li>\n <li>Excellent written and verbal communication skills. </li>\n <li>Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.</li>\n</ul>\n<p><strong> Education, Knowledge, Skills and Abilities Preferred</strong>: </p>\n<ul>\n <li>Master's of Science in Nursing </li>\n <li>Knowledge of insurance authorization process and system navigation </li>\n <li>Understanding of CPT and ICD-10 coding </li>\n</ul>\n<p><strong>Licenses and Certifications Required</strong>:</p>\n<ul>\n <li>NJ State Professional Registered Nurse License.</li>\n <li>AHA Basic Health Care Life Support HCP Certification. </li>\n</ul>\n<p><strong>Licenses and Certifications Preferred</strong>:</p>\n<ul>\n <li>Specialty specific and/or population certifications.</li>\n</ul>\n<p> </p>\n<p>If you feel that the above description speaks directly to your strengths and capabilities, then please apply today!  </p>\n<h2>Starting Minimum Rate</h2>Minimum rate of $92,144.00 Annually\n<h2>Job Posting Disclosure</h2>HMH is committed to pay equity and transparency for our team members.
