Responsible for collecting and managing data requested for outreach reporting.</li><li style=\"margin-left:0in;\">Conducts community presentations and trainings.</li><li style=\"margin-left:0in;\">Monitors outreach phone log and all team service documentation to ensure the completion of accurate and timely documentation of services provided.
