Monday through Friday - No Nights or Weekends</strong></p>\n<p> </p>\n<p><strong>For more information call Ann at 586-330-5223</strong></p>\n<p> </p>\n<p><strong>This is a great opportunity to expand on your leadership skills.  </strong></p>\n<p> </p>\n<p><strong>What you will do as a Social Worker Supervisor:  </strong>Plan, direct, define and coordinate individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance use and physical and emotional abuse.</p>\n<p> </p>\n<p><strong>Harbor Oaks Hospital is a leading provider for mental health services for children, adolescents, adults, and senior adults. </strong>We are dedicated to offering comprehensive care and support to individuals and families facing mental health challenges. </p>\n<p> </p>\n<p><strong>If you possess empathy, professionalism, and a commitment to patient well-being, we invite you to make a meaningful impact in our community.</strong></p>\n<p> </p>\n<h2>Responsibilities</h2>\n<p> </p>\n<p><strong>ESSENTIAL FUNCTIONS:</strong></p>\n<ul>\n <li>Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times.</li>\n <li>Through patient observation and patient/family interview obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan.</li>\n <li>Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle. </li>\n <li>Participate in multidisciplinary treatment team meeting to discuss psychosocial assessment, treatment planning and ongoing discharge planning.</li>\n <li>Identify, assess and coordinate patient cases and anticipate their needs related to illness, hospitalization, finances, education and home/community services.</li>\n <li>Facilitate education, process groups, support groups and referrals as requested.</li>\n <li>Conduct intakes and assist in coordinating placements within facility programs as required.</li>\n <li>Develop and execute clinically relevant activities in coordination with activity professionals, as required.</li>\n <li>Promote recovery, symptom reduction and the achievement of the highest level of social functioning.</li>\n <li>Communicate or aid family members in understanding and supporting the patient.</li>\n <li>Manage a caseload and assume responsibility for patient chart documentation.</li>\n <li>Facilitate education, process groups, support groups and referrals as requested.</li>\n <li>Manage a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards.</li>\n <li>Supervise social work interns completing field placement, as required.</li>\n <li>Provide direction and guidance to social services staff or other staff members, as required.</li>\n <li>Lead the program/unit and coordinate the activities of other clinical or direct patient care staff, providing direction and clinical guidance, as required.</li>\n <li>Demonstrate a positive, empathetic and professional attitude towards customers always.  When patient needs are not met, acknowledge and work to resolve complaints.  Recognize that patient safety is a top priority.</li>\n</ul>\n<p><strong>OTHER FUNCTIONS:</strong></p>\n<ul>\n <li>Perform other functions and tasks as assigned.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:</strong></p>\n<ul>\n <li>Master's degree in Social Work from an accredited college or university required.</li>\n <li>Three or more years' related experience working with comparative patient population required.</li>\n <li>Supervisory experience preferred if position is responsible for supervising staff.</li>\n</ul>\n<p><strong>LICENSES/DESIGNATIONS/CERTIFICATIONS: </strong></p>\n<ul>\n <li>State licensure, as required by the state in which the facility operates; which could include: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW)</li>\n <li>CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).</li>\n <li>First aid may be required based on state or facility.</li>\n</ul>\n<p>                                                                                                                                                                                                                </p>\n<p><strong><em>While this job description is intended to be an accurate reflection of the requirements of the job, management reserves the right to add or remove duties from particular jobs when circumstances (e.g. emergencies, changes in workload, rush jobs or technological developments) dictate.</em></strong></p>\n<p> </p>\n<p> </p>\n<p>We are committed to providing equal  employment opportunities to all applicants for employment regardless of an individual’s characteristics protected by applicable state, federal and local laws.</p>\n<p> </p>\n<p>HOAK01</p>\n<p>#LI-HOH</p>",
