    "ExternalDescriptionStr": "<p>We are seeking an exceptionally skilled RN Case Manager to join our dedicated Case Management team. This position offers an outstanding opportunity to work in a dynamic inpatient setting where you will plan and coordinate patient care from pre-hospitalization through discharge. You will hold a meaningful role in ensuring flawless care and treatment, collaborating closely with all members of the health care team.</p>\n<p>Responsibilities</p>\n<ul>\n <li>Perform evaluations and assessments within the established timeframes</li>\n <li>Detail appropriate reviews for assigned patients using our utilization review tool</li>\n <li>Provide telephonic reviews for identified contracted/private patients and collaborate with on-site and/or outside reviewers</li>\n <li>Keep patients informed of progress and provide information related to disease progression</li>\n <li>Collaborate with discharge planners to make orders and arrange for home care equipment and healthcare needs</li>\n <li>Work with third-party payers to validate orders</li>\n <li>Educate patients and families on all aspects of hospitalization and continuing care</li>\n <li>Assume responsibility for timely completion of required case management reports for regulatory bodies, health plans, and insurance carriers</li>\n <li>Interact professionally with patients, families, and caregivers, involving them in the formation of the plan of care and discharge needs</li>\n <li>Coordinate with the multidisciplinary team to ensure the identification of a safe and appropriate discharge plan for each assigned patient</li>\n <li>Ensure documentation meets current standards and policies</li>\n <li>Maintain department cleanliness and safety</li>\n <li>ONSITE POSITION</li>\n</ul>",
