Provide education and skills building in independent living, prevocational, academic, social, and communication skills to participants in group setting.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tAssess, evaluate and monitor the safety of individuals and others at all times.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tImplements individual behavioral modification and safety plans when applicable.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tDevelop person-centered, individualized treatment plans based on recommended goals identified in the individual&rsquo;s assessment.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tComplete all required documentation in a timely manner which includes but not limited to, the tracking of all individualized behaviors, individual service plans, monthly reports, quarterly reports, discharge planning, progress notes and all related documentation in compliance with DMAS, Licensure and Dominion Care standards.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tEngage in Care Coordination with other staff and community service providers to ensure the appropriateness of treatment modalities and services being provided. Attends supervision and staff meetings to address the individual&rsquo;s needs/progress and any administrative needs.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tProvide 24-hour emergency response as needed.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tAttends all meetings related to case management needs of individuals (IAP&rsquo;s, Med Evals, FAPT, psychological testing, etc.).&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tMaintains contact with program leadership and referral agencies on a regular basis to discuss updates, progress, concerns, or provide feedback.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tDevelops and maintains a network of supportive community referrals for individuals.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tRefers and links individuals to community resources.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tEstablishes working relationships with representatives of other agencies who work with and provide services to individuals.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\n\\n\\t\\n\\tEnsures the appropriate level of service is provided to each individual.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tMaintains compliance with all Licensure and Medicaid Standards.&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tAdhere to all Dominion Care policies and procedures and complete other tasks which may&nbsp;\\n\\t\\n\\n\\n\\n\\n\\n\\t\\n\\tbe necessary for the successful management of the program.&nbsp;\\n\\t\\n\\n\\n\\n\\n&nbsp;\\n\\n\\n\\nAbout the Company: At Dominion Care, founded in 1999, Care is what we do.
