In addition to direct patient care activities, this position is responsible for ensuring appropriate medication therapy and clinical outcomes for the institution related to specialty area.</p>\n<ul>\n <li>Practices as a clinical expert in specialty practice area.</li>\n <li>Develops and promotes advanced clinical practice based upon recent evidenced based practice.</li>\n <li>Practices under existing and establishes new collaborative practice agreements to enhance pharmaceutical care.</li>\n <li>Ensures institution-wide appropriate therapy and institutional outcomes in specialty area.</li>\n <li>Consistently provides patient-centered, interdisciplinary pharmaceutical care.</li>\n <li>Proactively reviews and processes orders accurately and efficiently.</li>\n <li>Prevents and/or resolves drug-related problems to ensure safety, efficacy, and cost-efficiency by consulting with the ordering prescriber and appropriately documents.</li>\n <li>Performs daily in-depth patient profile review to assess, monitor and recommend appropriate drug therapy as applicable.</li>\n <li>Consistently evaluates, intervenes, and documents clinical interventions beyond institutional protocols and guidelines.</li>\n <li>Supports recommendations with critical analysis of literature.</li>\n <li>Monitors drug therapy regimens and makes recommendations or modifications per protocol.</li>\n <li>Leads the development of system nomograms and other guidelines.</li>\n <li>Promotes the documentation of clinical activities and adverse drug events.</li>\n <li>Attends medical codes as applicable.</li>\n <li>Develops system policies and processes for proper dosing, storage, handling, dispensing, and/or administration of medications and compounded products.</li>\n <li>Actively participates in development of system clinical practice initiatives.</li>\n <li>Serves as a role model for complex clinical and ethical situations and resolves conflict constructively.</li>\n <li>Develops, uses and promotes effective patient education materials and techniques for drug therapy, adverse effects, compliance, appropriate use, handling and drug administration education.</li>\n <li>Participates in coordination of transitions of care as appropriate.</li>\n <li>Responsible for service completion and patient satisfaction.</li>\n <li>Engages in external professional activities to maintain clinical competency and knowledge of current trends and practices.</li>\n <li>Develops accredited continuing education activities every 2 years.</li>\n <li>Precepts pharmacy students and residents as available.</li>\n <li>Responsible for department training and competency assurance for specialty area.</li>\n <li>Develops and assists in training and education of pharmacy and other disciplines.</li>\n <li>Assists pharmacists seeking to advance their clinical skills.</li>\n <li>Answers drug information questions with in-depth knowledge of research designs, methodologies, and biostatistics.</li>\n <li>Actively participates in scholarly activities in specialty area; Submits publications or national presentations routinely.</li>\n <li>Recognizes process failures and actively engages in analysis and improvement efforts.</li>\n <li>Leads medication use evaluation or quality improvement project annually in specialty area.</li>\n <li>Oversees the work performed by pharmacy students, technicians and pharmacists to ensure performance standards are maintained and work is in accordance with state, federal and organizational regulations.</li>\n <li>Documents adverse drug reactions and medication variances per policy.</li>\n <li>Reviews trends and recommends patient safety solutions in specialty.</li>\n <li>Promotes new ideas to improve quality or overcome barriers to advance the profession in specialty area.</li>\n <li>Responsible for overall departmental pharmacy financial stewardship for specialty area.</li>\n <li>Leads the development and reinforcement of medication use policies and processes to ensure financial medication stewardship.</li>\n <li>Actively participates in formulary review and medication selection to promote cost savings.</li>\n <li>Supports and promotes cost-savings initiatives.</li>\n <li>Engages in review of medication insurance authorization status as applicable.</li>\n <li>Maintains and promotes understanding of 340b practices as applicable.</li>\n <li>Responsible for oversight of medication use systems in specialty area.</li>\n <li>Leads the development and review of protocols, guidelines, or order sets.</li>\n <li>Participates in multidisciplinary or system Emory Healthcare committee.</li>\n <li>Performs other duties as assigned or requested.</li>\n</ul>\n<p>MINIMUM QUALIFICATIONS:</p>\n<ul>\n <li>Graduate of an accredited school of pharmacy.</li>\n <li>ASHP- Accredited Post Graduate Year 1 and 2 Residency or Fellowship or equivalent experience in the specialty area.</li>\n <li>Relevant Pharmacy Specialty Board Certification is preferred.</li>\n <li>Eligible for Georgia Licensure. Candidate is required to be licensed in Georgia prior to official start date.</li>\n <li>Basic Life Support (BLS) Certification in accordance with the American Heart Association required within 90 days.</li>\n <li>Advanced Cardiac Life Support required within 6 months, if directly applicable to practice specialty.</li>\n</ul>\n<p>PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks. ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock , Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation , Shift work, Travel may be required.
