font-family: Arial, sans-serif; color: rgb(65, 65, 65);\">At Cornerstone Hospice, The Clinician Admrole is responsible for all activities designated to enhance and coordinate relationships with hospital referral sources, increase referrals, and assist in timely admissions, essentially in the hospital environment<span style=\"background:white;\">, primarily in the Southlake hospital.&nbsp;</span></span></p><p style='margin-right:0in;margin-left:0in;font-size:13px;font-family:\"Calibri\",sans-serif;margin:0in;margin-bottom:11.25pt;margin-top:0in;background:white;'><span style=\"font-size: 18px; font-family: Arial, sans-serif; color: rgb(65, 65, 65);\"><span style=\"background:white;\">These activities may include but are not limited to: medical community educational activities, patient referral functions, general inpatient admissions (GIP), case management of hospitalized team patients, discharge planning and assistance with admission activities. &nbsp;The Clinician bears ultimate responsibility for the case management of newly admitted GIP patients and hospitalized team patients. &nbsp;This includes assuring diagnostic tests, treatments, and symptom management issues the patient is experiencing are related to their hospice diagnosis. &nbsp;The Clinician further facilitates timely discharge to the appropriate level of care.</span></span></p><p id=\"isPasted\"><span style=\"font-family: arial, sans-serif; assures communication and collaboration between all involved agencies facilities, physicians and staff. &nbsp;</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 18px;\">Manages general inpatient admissions and hospitalized team patients while in the hospital, providing case management of patient care, including coordination of appropriate palliative symptom management with ordering physicians and hospital case managers. &nbsp;Assures appropriateness of orders and treatments and effective symptom control while managing length of stay and patient inpatient throughput.&nbsp;</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 18px;\">Notifies ordering physician and hospital case manager staff that patient is hospitalized for management of symptoms related to Hospice diagnosis that patient care is being covered under their election of the hospice benefit.
