Description

$10,000 Sign On Bonus At Cornerstone Hospice, The Clinician Admrole is responsible for all activities designated to enhance and coordinate relationships with hospital referral sources, increase referrals, and assist in timely admissions, essentially in the hospital environment , primarily in the Southlake hospital. These activities may include but are not limited to: medical community educational activities, patient referral functions, general inpatient admissions (GIP), case management of hospitalized team patients, discharge planning and assistance with admission activities. The Clinician bears ultimate responsibility for the case management of newly admitted GIP patients and hospitalized team patients. This includes assuring diagnostic tests, treatments, and symptom management issues the patient is experiencing are related to their hospice diagnosis. The Clinician further facilitates timely discharge to the appropriate level of care. The Admission APRN role is primarily based in the patient’s home, ALF or LTC. They may assist Clinicians in the hospital setting in obtaining consents and completion of Guidelines. Both roles are responsible for determining the eligibility and preparation of patient/families for hospice care. The admissions process includes establishing a working relationship with the patient/family, the development of the initial assessment of patient/family needs and the initial written plan of care. Manages general inpatient admissions and hospitalized team patients while in the hospital, providing case management of patient care, including coordination of appropriate palliative symptom management with ordering physicians and hospital case managers. Assures appropriateness of orders and treatments and effective symptom control while managing length of stay and patient inpatient throughput. Notifies ordering physician and hospital case manager staff that patient is hospitalized for management of symptoms related to Hospice diagnosis that patient care is being covered under their election of the hospice benefit. Notifies patient and patient physician when care/testing/treatment being ordered is outside the scope of the Hospice plan of care and the impact of such decisions on their Hospice benefit. Seeks opportunities to expand Hospice utilization within hospitals in critical care units, emergency departments, outpatient services and other departments and settings as may arise. Works to discharge patients to pre-hospitalization level of care if possible and appropriate to patient condition, considering patient and family choices. Provides Hospice information to patients/families, prepares them for admission, initiates the admission process, including principles of effective symptom management, and completes admission paperwork including verbal and written explanation of Patient Rights and Responsibilities, and any other information or consents required by regulation. Completes accurate documentation within the EMR system at the time of care and location of service. Where not possible or otherwise directed, completes at next available opportunity, and by end of shift without exception Performs a physical/environmental assessment, in addition to a psychosocial and spiritual assessment if an admissions social worker is not involved. Provides primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process. Obtains signatures on all necessary consents ensuring that patient/families are informed of their rights and responsibilities. Documents assessments and establishes the written initial plan of care and plan of treatment. Qualifications Florida A.P.R.N. license required; BSN, Master’s degree or CHPN preferred. Three years’ field experience or two years’ Hospice experience or equivalent. Excellent oral and written skills. Excellent assessment skills with demonstrated ability in physical assessments. Understanding of the dynamics of human behavior and the ability to communicate with people in a crisis. Personable, strong initiative and ability to work independently. Previous teaching/sales experience a plus. Must be able to work flexible hours. Valid Florida driver’s license and required auto liability insurance required.
