    "requisitionDescription": "<div><div><p>&nbsp;<span style=\"font-size: 12pt;font-family: Arial, sans-serif;\">POSITION SUMMARY:</span>&nbsp;</p><p><span style=\"font-size: 12pt;font-family: Arial, sans-serif;\">The Population Health Nutrition Specialist works collaboratively with other programmatic and clinical specialists assigned to respond to new value based payment models supported by Hawaii Managed Care Organizations.&nbsp;</span> <span style=\"font-size: 12pt;font-family: Arial, sans-serif;\">The nutrition specialist assesses nutrition and diet health needs of patients at high risk of chronic disease and provides clinical counseling on nutrition issues and healthy eating habits in a culturally sensitive manner. The nutrition specialist addresses gaps in care in patients identified as benefitting from clinical counseling related to food, nutrition and diet concerns while being mindful of social determinants of health, specifically food insecurity and food access. The position requires knowledge of preventive and disease intervention strategies related to patients with chronic disease as well as those with co-morbid conditions including social and behavioral influences.
