Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p>We are seeking a <strong>Remote House Calls Nurse Practitioner</strong> in NJ, OH, PA, NE, TX to support our in-home medical services program. This role provides <strong>virtual clinical oversight, patient assessments, care coordination, and provider support</strong> for patients receiving care in their homes. The ideal candidate is comfortable practicing telehealth, managing complex chronic conditions, and collaborating with field-based clinicians. </p>\n<p>This is a fully remote position with a strong focus on quality, efficiency, and patient-centered care. </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Conduct <strong>telehealth visits </strong>for patients enrolled in the House Calls program </li>\n</ul>\n<ul>\n <li>Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services </li>\n</ul>\n<ul>\n <li>Perform <strong>care management needs assessments, follow-ups, and care plan adjustments</strong> </li>\n</ul>\n<ul>\n <li>Review patient histories, labs, imaging, and diagnostic results </li>\n</ul>\n<ul>\n <li>Support <strong>medication management and reconciliation</strong> </li>\n</ul>\n<ul>\n <li>Coordinate care with primary care providers (if patients already have one), specialists, and care management teams </li>\n</ul>\n<ul>\n <li>Document visits and clinical decisions accurately in the EMR </li>\n</ul>\n<ul>\n <li>Ensure compliance with clinical protocols, quality standards, and regulatory requirements </li>\n</ul>\n<ul>\n <li>Participate in interdisciplinary case reviews and quality improvement initiatives </li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Active <strong>Nurse Practitioner (NP) license</strong> in the required state(s) </li>\n</ul>\n<ul>\n <li>National NP certification (ANCC or AANP) </li>\n</ul>\n<ul>\n <li>Minimum <strong>2+ years of clinical experience</strong> as a Nurse Practitioner </li>\n</ul>\n<ul>\n <li>Experience in <strong>house calls, home health, geriatrics, primary care, or internal medicine</strong> strongly preferred </li>\n</ul>\n<ul>\n <li>Prior <strong>telehealth or remote care experience</strong> preferred </li>\n</ul>\n<ul>\n <li>Strong clinical judgment and comfort managing complex patients remotely </li>\n</ul>\n<ul>\n <li>Excellent communication and documentation skills </li>\n <li>Must be licensed in NY and one of the other states listed or willing to obtain</li>\n</ul>\n<p> </p>\n<p><strong>Technical & Regulatory Requirements</strong> </p>\n<ul>\n <li>Proficiency with <strong>EMR/EHR systems</strong> </li>\n</ul>\n<ul>\n <li>Comfortable using telehealth platforms and remote monitoring tools </li>\n</ul>\n<ul>\n <li>Active <strong>DEA registration</strong> (or willingness to obtain, if applicable) </li>\n</ul>\n<h2>Equal Opportunity Employer</h2>\n<p><strong><em>Essen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.</em></strong></p>",
