Current benefits include:</p><ul><li>Medical, vision, and dental</li><li>Short-term and long-term disability options</li><li>A 401K retirement program</li><li>Paid time off and paid holidays</li></ul><p><br></p><p><strong>The Role:</strong></p><p>As a <strong>Referral Coordinator</strong>, plays a key role by scheduling referred patient appointments, tracking and scheduling circulation rides, and answering questions from community partners (including jails, emergency departments, treatment facilities, others).</p><p><br></p><p><strong>Additional responsibilities include:</strong></p><ul><li>Receives inbound referrals from community partners in company's CRM and EHR, via phone, chat, or email; processes and schedules, if appropriate.</li><li>Documents necessary information in patient charts.</li><li>Gathers new patient demographic and insurance information.</li><li>Follows up on patient inquiries.</li><li>Develop trusted relationships with community partners.</li><li>Understand nuances of community partner referrals and provide accurate responses.</li><li>Ability to transfer phone calls, provide phone messages and escalate to appropriate staff.</li><li>Route Ideal Option patient inmate prescription refill requests to prescribing provider.</li><li>Uses problem solving skills to implement solutions and escalate unresolved problems.</li><li>Represents Ideal Option professionally and with a positive attitude.</li><li>Participates in continuous quality improvement activities.</li><li>Comply with national Protected Health Information laws including HIPAA and 42 CFR Part 2.</li><li>Works on-call for one week once every 4-5 Weeks.</li><li>Other projects and duties as assigned.</li></ul><p>·</p><p><strong>Competencies:</strong></p><ul><li>Excellent customer service skills.</li><li>Typing speed of 60 words per minute or more.</li><li>General knowledge regarding use of office equipment.</li><li>Effective verbal and written communication skills especially conveying complex information in an understandable manner.</li><li>Proficient in CRM and EHR systems.</li><li>Ability to multitask, learn quickly, and work in a fast-paced environment.</li><li>Ability to work independently and as a team member.</li><li>Ability to establish and grow strong relationships.</li></ul><p><br></p><p><strong>Our ideal candidate:</strong></p><p>Has effective verbal and written communication skills.
