    "description": "<p>EMERGENCY DEPARTMENT PHARMACIST</p>\r\n\r\n<p>&nbsp; (<strong>Must have Completed &nbsp;an ASHP accredited PGY-1 Pharmacy Practice Residency to apply No Exceptions!</strong>)</p>\r\n\r\n<ul>\r\n\t<li>Work collaboratively with multidisciplinary teams to optimize drug therapy.</li>\r\n\t<li>Provide direct patient care, including treatment assessment and medication management activities.</li>\r\n\t<li>Clinically evaluate all patients in specified area on a daily basis with relevant documentation placed in the medical record recommendations, and Interpret medication orders, profile orders and oversee the packaging, labeling and distribution of medications.</li>\r\n\t<li>Perform drug therapy monitoring and intervene as necessary on drug dosing, drug administration, drug interactions, side- effects and monitoring issues; complete therapeutic drug monitoring consults and adverse drug reaction reports.</li>\r\n\t<li>Provide accurate and timely drug information to physicians, nurses and other health professionals.</li>\r\n\t<li>Respond to level 1 trauma, code blue massive blood transfusion emergencies.</li>\r\n\t<li>Provide direct patient care and medication therapy monitoring, including antibiotic stewardship, pharmacokinetic drug dosing, renal and hepatic dosing, and patient education.</li>\r\n\t<li>Develop, implement, and monitors clinical protocols.</li>\r\n\t<li>Provides consultative services and clinical expertise to evaluate and improve medication usage and therapeutic outcomes.</li>\r\n\t<li>Process orders and prescriptions to provide medications and pharmaceutical supplies ordered by physicians and authorized medical practitioners.</li>\r\n\t<li>Facilitate discharge planning and smooth transition to alternate sites of care from a pharmacy perspective.</li>\r\n\t<li>Maintain and updates a patient profile with demographics, diagnosis, allergies, and current medications.</li>\r\n\t<li>Discuss medication order clarifications with the prescriber, document clinical interventions and cost avoidance as part of the departmental documentation program.</li>\r\n\t<li>Educate patients on discharge medications.</li>\r\n\t<li>Provide cost-effective management of drug utilization within the clinic.</li>\r\n\t<li>Monitor process and outcome of patient care relating to drug therapy.</li>\r\n\t<li>Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.</li>\r\n</ul>\r\n",
