font-size: 14px;\">The<strong>&nbsp;<u>Per Diem Physician</u>&nbsp;</strong>provides care, including routine health maintenance, management of<strong>&nbsp;</strong>chronic conditions, and management of acute illness for patients based upon a per diem schedule arranged by the Site Medical Director.<br><br><strong>Responsibilities:&nbsp;</strong></span></p><ul id=\"isPasted\"><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Provides primary care coverage and care coordination to those patients who have primary care providers at the health center location.</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Provides primary care to include but is not limited to outpatient chronic care and acute care visits, routine health maintenance visits as well as outpatient telehealth visits.</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Provides primary care coordination to include but is not limited to referral for specialty care, coordination of hospital follow-up visits.</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">While scheduled in clinic, manages laboratory results that become available during the hours of their scheduled shift.</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">While scheduled in clinic, supports in-basket coverage for PCPs not on site. &nbsp;</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Acts as a rapid response MD available if another PCP or team anchor is not available.</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Supervises mid-level practitioners who are working alongside the per diem provider in clinic.</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Supports patients in managed care plans and assists patient understanding of such plans, including attention to quality improvement care gaps. &nbsp;&nbsp;</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Complies with state continuing education laws. &nbsp;</li><li style=\"font-family: verdana, sans-serif; font-size: 14px;\">Provides credentialing information on a timely basis.</li><li style=\"font-family: verdana, sans-serif;
