    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><table border=\"1\" cellspacing=\"0\" id=\"isPasted\"><tbody><tr><td><p>Position Summary:</p><p>Foster and maintain ongoing relationships with family members, service providers, hospitals, courts, education system providers, and other identified resources to facilitate and implement the Wraparound process.&nbsp;</p></td></tr><tr><td><p>Position Description:</p><ul><li>Cultivate flexible, family-focused, community-based responsive services</li><li>Develop and maintain the Wraparound Team, including coordinating and leading team meetings</li><li>Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary</li><li>Ensure family support and stabilization during crises</li><li>Provide and document the initial and ongoing Life Domain Assessment</li><li>Maintain all service documentation requirements, evaluation outcome requirements and data as required</li><li>Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan</li><li>Utilize and monitor Flexible Funding and service coordination</li><li>Obtain weekly reports from subcontracted providers</li><li>Participate in after hours on-call response</li><li>Attend Program staff meetings, supervision and any other meetings as required</li><li>Participate in the Agency and Program CQI Peer review process</li><li>Perform duties to reflect Agency policies and procedures and comply with regulatory standards</li><li>Meet Agency training requirements</li><li>Report all MUIs to Site Manager and Supervisor immediately</li><li>Other duties as assigned</li></ul><table border=\"1\" cellspacing=\"0\" id=\"isPasted\"><tbody><tr><td><p>Required Knowledge, Skills and Abilities:</p><ul><li>Knowledge of community-area resources for youth and families</li><li>Effective written and oral communication skills, including meeting facilitation</li><li>Ability to manage time and multiple priorities effectively</li><li>Demonstrated organizational abilities</li><li>Ability to work effectively with individuals of a diverse background</li></ul></td></tr><tr><td><p>Job Requirements:</p><ul><li>Bachelors degree in related field and two years of direct experience or five years of related experience</li><li>Must complete High Fidelity Wraparound model training within 12 months of employment.&nbsp;</li></ul></td></tr><tr><td><p>Essential Functions/Physical Demands:</p><ul><li>Positional: Driving in accordance with job duties assigned.
