    "ExternalDescriptionStr": "<p><strong><u>POSITION SUMMARY</u></strong></p><p>Under the supervision of the Director, Population Health/Case Management, the Ambulatory Care Manager Navigator – RN is responsible for assessing and identifying psychosocial needs for the Mount Nittany Physician Group patient population.<span>&nbsp; </span>Manages high risk/high cost patients by creating a care plan, providing education, psychosocial support, and connecting patients to community resources.<span>&nbsp; </span>Effectively communicates plan to Providers.<span>&nbsp; </span>Continues to assess patient’s progress on care goals with regular and ongoing communication. Develops and maintains relationships with community based organizations.</p><p>LEADERSHIP VALUES</p><p><strong><u>MINIMUM REQUIREMENTS</u></strong></p><p><br><strong><u>Education:</u></strong></p><ol style=\"padding-left: 24px;\"><li>Master of Science in Nursing with a minimum of two years’ experience in a health care setting. <strong>OR</strong></li><li>Bachelor of Science in Nursing with a minimum of four years current experience in a health care setting.</li><li>An Associate Degree or Diploma RN with a minimum of eight years clinical experience and four years care manager or case manager experience will be considered.</li></ol><p><strong><u>Experience:</u></strong></p><ol style=\"padding-left: 24px;\"><li><span style=\"color: rgb(51, 51, 51);\">Preference is given to previous case management experience.</span></li></ol><p><strong><u>Knowledge, Skills, Abilities:</u></strong></p><ol style=\"padding-left: 24px;\"><li><span>Ability to effectively assess and communicate with patients, their families, providers, and outside agencies.</span></li><li><span>Ability to independently work and problem solve based on patient’s needs.</span></li><li><span>Strong interpersonal skills; ability to communicate effectively with all levels of management and staff across the System.</span></li><li><span>Exceptional customer service and demonstrated effective interaction with physicians, patients, and staff.</span></li><li><span>Oral and written communication skills; Strong diplomacy and collaboration skills.</span></li><li><span>Sound decision-making skills.</span></li><li><span>Travel to office locations is expected.</span></li></ol><p><strong><u>License/Certification/Registration:</u></strong></p><ol style=\"padding-left: 24px;\"><li><span style=\"color: black;\">If incumbent is a Registered Nurse, must be licensed as a Registered Nurse in the state of Pennsylvania.&nbsp;</span></li><li>Case Management Certification preferred.</li></ol><p><strong><u>SUPERVISION RECEIVED</u></strong></p><p><span style=\"letter-spacing: -0.1pt;\">Receives supervision from the Director, Population Health/Case Management.
