Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p><strong>Job Summary:</strong> The Physician Assistant (PA) specializing in house calls provides high-quality medical care to patients in their homes, particularly focusing on individuals with chronic illnesses, limited mobility, or barriers to accessing traditional healthcare. This role emphasizes patient-centered care, effective communication, and collaboration with other healthcare professionals.</p>\n<h2>Responsibilities</h2>\n<p><strong>Responsibilities include:</strong></p>\n<ul>\n <li><strong>Patient Assessments:</strong> Perform thorough evaluations of patients’ health status, including physical examinations, health histories, and medication reviews during home visits.</li>\n <li><strong>Diagnosis and Treatment:</strong> Diagnose medical conditions and develop personalized treatment plans, prescribing medications and therapies as appropriate.</li>\n <li><strong>Care Coordination:</strong> Collaborate with physicians, nurses, and other healthcare team members to ensure comprehensive and continuous care for patients.</li>\n <li><strong>Patient Education:</strong> Educate patients and their families on disease management, medication adherence, preventive care, and lifestyle changes to enhance health outcomes.</li>\n <li><strong>Documentation:</strong> Maintain accurate and detailed records of patient encounters, treatment plans, and follow-up care in electronic health records.</li>\n <li><strong>Follow-Up Care:</strong> Schedule and conduct follow-up visits to monitor patients' progress, adjusting treatment plans as necessary.</li>\n <li><strong>Emergency Management:</strong> Assess and address urgent medical needs during home visits, coordinating referrals and additional support when required.</li>\n <li><strong>Community Outreach:</strong> Engage in community health initiatives to promote awareness of home healthcare services and available resources.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Job Requirements: </strong></p>\n<ul>\n <li>Master’s Degree required</li>\n <li>Active New York license as a Physician Assistant</li>\n <li>Currently New York Controlled Substance and Drug Enforcement Registration</li>\n <li>Master's degree</li>\n <li>Graduate of an approved PA Program and a post-graduate training program in surgery or at least one-year equivalent work experience</li>\n <li>Current Certification by the National Commission on Certification of Physician Assistants (NCCPA)</li>\n <li>Emergency Medicine / Family Practice/Acute Care</li>\n <li>DOT, BLS, ACLS Preferred</li>\n <li><strong>Bilingual a plus - Spanish</strong></li>\n <li>Proven work experience as a physician assistant or nurse</li>\n <li>Knowledge of EMR Systems preferred</li>\n</ul>\n<p><strong>Work Environment:</strong></p>\n<ul>\n <li>Primarily operates in patients’ homes, which may vary in condition and accessibility.</li>\n <li>Requires travel within a designated geographic area, with flexibility to accommodate patient needs, including potential evening and weekend visits.</li>\n <li>Collaborative work with a multidisciplinary team may occur remotely or in various healthcare settings.</li>\n</ul>\n<p>Salary: $135,000-$200,000</p>\n<p>(May include bouses and incentives) </p>\n<h2>Equal Opportunity Employer</h2>\n<p><strong><em>Essen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.</em></strong></p>",
