Join DCI today to build relationships and gain fulfillment serving individuals in our comfortable clinical setting with a lower caregiver-to-patient ratio than other providers.</p>\n<p> </p>\n<p>The <strong>Dialysis Social Worker</strong> is responsible for assessing and responding to the psychosocial needs of our dialysis patients to ensure the best possible outcomes and improve quality of life. </p>\n<p> </p>\n<p>This is a temporary position, up to 6 months.</p>\n<p> </p>\n<p><strong>Schedule:</strong> Fulltime Monday through Friday, shifts start at 8am, weekends off</p>\n<p> </p>\n<p><strong>Compensation</strong>: Pay range from $38.00-$48.00 per hour, depending on dialysis experience</p>\n<p> </p>\n<p><strong>Benefits:</strong></p>\n<ul>\n <li>Support and mentorship opportunities available through peer and corporate training for individuals new to renal care</li>\n <li>Comprehensive medical, dental and vision benefits</li>\n <li>Life and long-term disability insurance provided at no additional expense to employee</li>\n <li>Paid time off (PTO) including holidays</li>\n <li>Extended Sick Bank (ESB) in addition to PTO – paid time for doctor appointments, sickness or medical leave</li>\n <li>Retirement plans with $.50 of each contributed dollar matched for eligible employees, up to 8 percent</li>\n <li>Employee assistance program</li>\n <li>Wellness program</li>\n <li>Among others</li>\n</ul>\n<h2>Responsibilities</h2>\n<p><strong>What You Can Expect:</strong></p>\n<ul>\n <li>Provide patient support to address emotional, financial and lifestyle changes associated with chronic kidney disease</li>\n <li>Assist patients with utilizing community resources for nutritional, emotional, transportation and housing support</li>\n <li>Advocate and assist patients with accessing healthcare and other benefits</li>\n <li>Educate patients and their families regarding end-stage renal disease treatment options, including dialysis, transplant and medical management</li>\n <li>Help patients compare treatment options, empowering them to make informed decisions about their care</li>\n <li>Coordinate translation services for patients and caregivers</li>\n <li>Assess family and support system and refer for additional services as needed</li>\n <li>Provide counseling services to patients and their families directed at helping them cope with kidney failure and dialysis, follow the treatment plan and achieve the patient’s rehabilitation goals</li>\n <li>Develop and implement individualized care plans with interdisciplinary care team based on psychosocial assessment to help patients enjoy a better quality of life</li>\n <li>Collaborate with interdisciplinary care team to assess ongoing needs and modify care plan as necessary</li>\n <li>Educate interdisciplinary team members about emotional aspects of dialysis</li>\n <li>Advocate for patients at clinic, community and state/federal levels</li>\n <li>Serve as ESRD Network liaison</li>\n <li>Engage in a data-driven quality improvement process to positively impact outcomes</li>\n</ul>\n<h2>Qualifications</h2>\n<strong>Successful Candidates Bring:</strong>\n<ul>\n <li>Excellent communication skills</li>\n <li>Active listening skills to understand patient and family concerns</li>\n <li>Desire to collaborate with care teams</li>\n <li>Ability to problem solve</li>\n</ul>\n<strong>Education/Training:</strong>\n<ul>\n <li>Master’s Degree in Social Work (MSW) required</li>\n <li>Licensed in the state of practice as required by state regulations</li>\n <li>New MSW graduates welcome, job shadow opportunities available</li>\n <li>Renal care experience preferred, not required</li>\n</ul> \n<strong>DCI’s Differentiator:</strong>Since opening the first clinic 50 years ago in Nashville, Tenn., our Dialysis Clinic, Inc. family has grown to be the nation’s largest nonprofit dialysis provider with more than 270 locations in 30 states, serving nearly 14,000 patients each day.
