Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care. Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.</p>\n<h2>Qualifications</h2>\n<ul>\n <li>Must possess a high school diploma or equivalent. </li>\n <li>Must either 1) be a licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting; or 2) have at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services. </li>\n <li>Must have basic demonstrated technology skills, including operation of a mobile device.</li>\n</ul>\n<p>Education and experience, preferred</p>\n<ul>\n <li>Previous experience in home health, hospice, or pediatrics is preferred.</li>\n</ul>\n<p>Requirements*</p>\n<ul>\n <li>Must possess a valid state driver license</li>\n <li>Must maintain automobile liability insurance as required by law</li>\n <li>Must maintain dependable transportation in good working condition</li>\n <li>Must be able to safely drive an automobile in all types of weather conditions<em>* For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license</em></li>\n</ul>\n<h2>Additional Information</h2>\n<p><em>Enhabit Home Health & Hospice is an equal opportunity employer.
