    "requisitionDescription": "<div><div><div><div><div><div><div><div><p><strong>What you&rsquo;ll be doing:</strong></p><p id=\"isPasted\"><br></p><p>&bull; As a registered nurse in the Clinical Nurse Liaison role, you will provide quality care to patients from diverse socioeconomic and cultural backgrounds.</p><p>&bull; You&rsquo;ll be developing relationships with referral sources like specific providers, hospitals, long term care facilities, or doctor offices to enhance the admission process and provide community support. To do so you&rsquo;ll need to be:</p><p>- Familiar with marketing and strategic goals for the company.</p><p>- Be confident and knowledgeable about the services offered by Lower Cape Fear LifeCare.</p><p>- Able to provide service recovery and excellent customer service skills to ensure satisfaction.</p><p>- Able to manage multiple tasks simultaneously and quickly pivot from one task to another as needed.</p><p>- Understand hospice eligibility and advise on the most appropriate care settings.</p><p>&bull; Being comfortable with public speaking is vital, as you&rsquo;ll be conducting educational in-services with medical staff or community groups on topics such as:</p><p>- Hospice 101 &ndash; Eligibility and Philosophy</p><p>- End-of-Life and Symptom Management</p><p>- Specific Disease States (e.g., dementia, heart failure, COPD, end-stage renal disease)</p><p>- Medication usage in Hospice</p><p>&bull; While you will not be providing direct patient care as a registered nurse, you&rsquo;ll spend time working one on one with patients and their families to act as a consultant and providing education on identifying signs and symptoms, common medications used in hospice care, and what to expect at end-of-life.</p><p>&bull; You&rsquo;ll work to collaborate and coordinate with external medical staff and referral sources to determine patient eligibility for hospice, palliative care, or our Memory Partners program.</p><p>&bull; If patients meet admissions criteria, you&rsquo;ll work to coordinate transition of care into one of our three inpatient hospice care centers, in-hospital hospice, outpatient case managers to receive hospice care at home or hospice services in a facility setting.</p><p>&bull; This transition of care would include things like:</p><p>- Obtaining consent to treat from the patient or primary caregiver.</p><p>- Working with physicians and case managers on discharge planning.</p><p>- Coordinating with outside vendors, like transport companies, to organize patient pick up and arrival times.</p><p>- Completing documentation for new referrals and follow-ups after each visit.</p><p>- Working with LCFL staff to start the admissions process and order DME and other supplies.</p><p><br></p><p><strong>What we&#39;re looking for:</strong></p><p id=\"isPasted\">1.&nbsp;Education: &nbsp;Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program. &nbsp;Degree in business, marketing, or related field preferred.&nbsp;</p><p>2. Licensure / Certifications: &nbsp;Current license to practice professional nursing in the State of North Carolina or South Carolina depending on position location.&nbsp;</p><p>3. Experience: &nbsp;Two years experience. Prefer hospice or home care. &nbsp;Prefer experience in marketing, public affairs, community relations or hospital liaison nurse.&nbsp;</p><p>4. Essential Technical / Motor Skills: &nbsp;Ability to speak clearly to communicate with patients, families, physicians, and staff. &nbsp;Good command of the English language. Computer experience helpful.&nbsp;</p><p>5. 5. Interpersonal Skills: Ability to develop positive interaction with patients, families, physicians and staff and other health professionals to effectively communicate and educate about hospice services and end-of-life care. &nbsp; &nbsp;Strong verbal and written communication skills. &nbsp;Highly self-motivated and self-disciplined. &nbsp;Flexible.&nbsp;</p><p>6. Essential Physical Requirements: Ability to move freely. Ability to work long hours and manage stressful situations. &nbsp; &nbsp;</p><p>7. Essential Mental Abilities: Ability to define problems, collect data, establish facts, and draw valid conclusions. Requires higher level of mental faculties accompanied by short and long-term memory.&nbsp;</p><p>8. Essential Sensory Requirements: Touch, sight, hearing, ability to organize thoughts, facts, and ideas. &nbsp;Ability to translate the written word. &nbsp;&nbsp;</p><p>9. Exposure to Hazards: May be exposure to blood and body fluids, infectious diseases, needle puncture wounds. May encounter patients and other situations which present a potential threat to personal safety. &nbsp;</p><p>10. Hours of Work: Flexibility with schedule required. Weekends, holidays, evening, and early morning hours will be required from time to time. &nbsp;&nbsp;</p><p>11. Population Served: Physicians, RN&rsquo;s, facility administrators, discharge planners, social workers, other health professionals, insurance specialists, public, community leaders, vendors. &nbsp;All patient populations from infants through geriatric with life limiting illness. &nbsp;</p><p>12.
