What You'll Do
As a Senior Services Coordinator, you'll serve as the primary resource connector and advocate for residents across multiple CHP communities by:
Conducting resident assessments and developing individualized support plans
Connecting residents with healthcare, benefits, transportation, food assistance, and other essential services
Planning and facilitating wellness, educational, and social programs
Building partnerships with community organizations and service providers
Assisting residents with benefit applications and service navigation
Maintaining accurate documentation, reporting, and compliance requirements
Managing program resources and supporting community operations
Collaborating with property management and internal teams to improve resident outcomes

Qualifications
Required
High school diploma or equivalent and at least two years of experience in human services, social services, case management, resident services, or a related field; OR
Bachelor's degree in human services, social work, sociology, psychology, gerontology, or a related field with at least one year of relevant experience
Minimum one year of direct service experience supporting seniors and/or individuals with disabilities or special needs
Valid driver's license meeting CHP insurability requirements
