He/she provides the full scope of primary care services which fall under his/her field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance. The Nurse Practitioner is also a member of the greater Bread for the City team and furthers the Bread for the City mission in his/her daily work with clients and other staff. He/she works collaboratively within the Medical Clinic and across program areas toward the goals and objectives of the organization. The Chief Medical Officer provides guidance and direction to the Nurse Practitioner and is responsible for completing an annual performance review of the Nurse Practitioner.<strong>&nbsp;</strong></span></p><p><span style='font-family: \"times new roman\", serif; font-size: 14px;'><u><strong>Essential Responsibilities:</strong></u></span></p><p><span style='font-family: \"times new roman\", serif; font-size: 14px;'>1. Provides Bread for the City patients with primary care services within his/her field of training&nbsp;</span></p><ul type=\"disc\"><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Supports BFC&rsquo;s vision, mission, and goals in daily interactions with patients;</li><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Obtains patient health histories and perform physical examinations;</li><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Diagnoses, treat, and provide preventative care to all patients, and provide referrals where appropriate;</li><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Reviews incoming reports (e.g. lab, x-ray, EKG) and follow-up in a timely manner.</li></ul><p><span style='font-family: \"times new roman\", serif; font-size: 14px;'>2. Collaborates with a team of providers to ensure effective coordinated care for all patients and enable the continuous improvement of the Medical Clinic</span></p><ul type=\"disc\"><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Supervises students in training as assigned by the Chief Medical Officer;</li><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Complies with the Infection Control plan which includes personally utilizing universal precautions and ensures compliance by support staff;</li><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Participates in peer review, Quality Improvement/Quality Assurance committee meetings, provider meetings, and other clinical meetings;</li><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Provides support to other clinical staff as part of a team approach to provide quality care to patients;</li><li style='font-family: \"times new roman\", serif; font-size: 14px;'>Provides input and collaboration in Quality Improvement/Quality Assurance projects and policy/procedure creation.</li></ul><p><span style='font-family: \"times new roman\", serif; font-size: 14px;'>3.
