Assist individuals with career planning, which may include, but is not limited to interview preparation, exploring job openings, and completing steps to fulfill employer required documentation/training. Accurately complete individual attendance and billing sheets, ensuring there are service documentation notes to support all services being billed. Attend and participate in in-service, agency and funding meetings as required. Provide education, support, and training to employers regarding supervising people with disabilities. Maintain open and professional communication with outside agencies as relevant to individual needs. Exercise supervision of individual(s) at community-based worksites. Serve as Case Manager to assigned caseload and other individuals as necessary. This may include but is not limited to providing physical assessment; crisis intervention; individual counseling; independent living skills, leisure activities; skill training; job-seeking skill teaching; site development and modeling a variety of occupations and social skills. Support individuals to increase independence on the job and in community based on planning and support team service plan. Other related duties and responsibilities as assigned.
