Description
The Graduate Pharmacy Intern is a recent PharmD graduate actively pursuing pharmacist licensure. Under the direct supervision of a licensed pharmacist, this role supports clinical and operational pharmacy services to optimize medication use and patient outcomes. The intern may perform pharmacist-level functions as permitted by state law but may not independently verify prescriptions or make final clinical decisions.
Key Responsibilities
Assist in the safe, effective, and cost-efficient management of medications across the continuum of care. Review and evaluate medication orders for accuracy and appropriateness. Provide patient counseling on medication use, adherence, and side effects under pharmacist supervision. Support dispensing, monitoring, and communication of medication-related needs. Collaborate with pharmacists, providers, nursing, pharmacy technicians, and other medical staff to optimize patient care outcomes. Provide drug information and assist with clinical decision support. Identify and escalate medication-related problems and assist in developing care plans/solutions to medication-related problems. Support oversight of pharmacy technicians as directed. Maintain compliance with all policies, procedures, regulatory requirements, and professional standards. Participate in quality improvement initiatives and departmental goals. Maintain professionalism and a patient-centered approach in all interactions. Actively prepare for and obtain pharmacist licensure within established timelines Provide patient care services as needed, to include but not limited to: immunizations, medication therapy management, disease state consultations, adherence based patient calls Supervision
Reports to the Pharmacist-in-Charge (or designee). Works under the direct supervision of a licensed pharmacist.
