Learn more about our amazing facility at https://www.lecontemedicalcenter.com/about-leconte-medical-center/. </p>\n<p> </p>\n<p><strong>Position Summary:</strong></p>\n<p>Under general supervision; plans, evaluates and implements treatment programs for physical therapy patients.  Responsible for additional job responsibilities which may include:  program development, clinical coordination of students, scheduling of supportive staff and coordinating continuing education requirements.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Advanced clinical practice – including evaluation and specialized treatment of patients.\n  <ul>\n   <li>Performs patient evaluations (initial, progress, and discharge evaluations), establishes treatment plans, makes recommendations, and sets goals in accordance with patient needs and evidence based practice.</li>\n   <li>Meets established targets for passive/active treatment ratios</li>\n   <li>Involves patient and family in treatment, education and goal setting. Coordinates with other medical personnel regarding patient’s progress, needs and discharge planning.</li>\n   <li>Demonstrates knowledge and competency including providing for age-specific needs of the population served.</li>\n  </ul></li>\n <li>Serves as resource person for staff and senior staff on an ongoing basis.\n  <ul>\n   <li>Provides 8 hours education/mentorship time per year. This includes:\n    <ul>\n     <li>Peer counseling /Mentorship</li>\n     <li>Community lectures/Volunteer</li>\n     <li>Orientation of new staff in area of specialty</li>\n     <li>In-services/Case Studies</li>\n     <li>Lunch and Learns</li>\n    </ul></li>\n   <li>Development, implementation and evaluation of standards of care and evidence based practice:\n    <ul>\n     <li>Evidence base practice protocols</li>\n     <li>Clinical outcomes analysis and communication to team</li>\n     <li>New policy development</li>\n     <li>QI/QA</li>\n    </ul></li>\n  </ul></li>\n <li>Maintains and updates patient documentation in accordance with professional, departmental, organizational, and payer guidelines and timeframes. Maintains established targets for compliance with chart audits.</li>\n <li>Meets established targets for:\n  <ul>\n   <li>Quality/outcome measures</li>\n   <li>Service/patient satisfaction measures</li>\n   <li>Financial/productivity measures</li>\n   <li>Growth/patients-per-day measures</li>\n  </ul></li>\n <li>Demonstrates leadership, including but not limited to:\n  <ul>\n   <li>Development and implementation of new and current programs.</li>\n   <li>Participates in training, orientation and supervision of support staff.</li>\n   <li>Participates in orientation of new licensed staff in area of specialty</li>\n   <li>Acts as Clinical Instructor for at least one student per year as available.</li>\n   <li>Participates in peer evaluations for competency assessment.</li>\n   <li>Assists with continuing education courses hosted by Covenant Health.</li>\n   <li>Participates in professional organization/meetings (TPTA, APTA)</li>\n   <li>Volunteers for Covenant Health activities such as the marathon, golf tournament, sports physicals, employee education, etc.</li>\n  </ul></li>\n <li>Complete partnering activities per established targets per month.\n  <ul>\n   <li>Physician office visits</li>\n   <li>Community lectures/volunteer</li>\n   <li>Case Manager visits/marketing</li>\n   <li>Provide in-services</li>\n  </ul></li>\n <li>Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.</li>\n <li>Perform other related duties as assigned or requested.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Minimum Education:                        </strong></p>\n<p>None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.</p>\n<p> </p>\n<p><strong>Minimum Experience:                      </strong></p>\n<p>Four (4) years of experience with a minimum of three (3) years in specialty area.  </p>\n<p> </p>\n<p><strong>Licensure Requirement:                 </strong></p>\n<p>Must have and maintain Tennessee State Physical Therapy Licensure, with achievement of at least one of the following at time of hire or placement in the position:</p>\n<ul>\n <li>Advanced degree in specialty area</li>\n <li>Advanced degree associated with specialty area</li>\n <li>Professionally recognized certification in specialty area</li>\n <li>100 hours (10.0 CEU’s) of continued education</li>\n</ul>",
