Supervising and providing leadership and coaching to direct care staff within the community while ensuring compliance with state clinical regulations and requirements.&nbsp;</div><div><br></div><div id=\"isPasted\"><strong><span style=\"font-size: 24px;\">Work Schedule/ Shift</span></strong></div><div id=\"isPasted\">TBD</div><div><br></div><div id=\"isPasted\"><strong><span style=\"font-size: 24px;\">Compensation</span></strong></div><div id=\"isPasted\"><span id=\"isPasted\" style=\"font-size: 18px; font-style: normal; font-weight: 400; text-align: left; text-indent: 0px;\">Starting at</span>&nbsp;$77,900/annually</div><p><strong id=\"isPasted\"><span style=\"font-size: 24px;\">What you'll do</span></strong></p><ul><li id=\"isPasted\">Responsible for the direct supervision of community-based non-certified clinical care staff.</li><li>Provides training and education to care staff on an ongoing basis with classroom in-services and situation specific training.</li><li>Supervises the maintenance of resident charts and reviews documentation performed by licensed and unlicensed care staff.</li><li>As described and allowed in the Nurse Practice Act, assesses health, oversees medication administration, oversees functional and psycho-social status of residents, initiates individualized care plans, proactively manages care and services for each resident, evaluates effectiveness, and maximizes the resident’s opportunity to remain in their environment.</li><li>Manages the healthcare of residents, including the delivery of pertinent care information to families and staff.</li><li>Participates in the pre-admission process for new prospective residents. Ensures that all required documentation is completed prior to resident admission including nursing assessments, care plans and other associated forms.</li><li>Documents and evaluates changes in condition and notifies Executive Director, resident physician and resident’s legally responsible party of resident’s condition.</li><li>Provide oversight for community’s medication management program to ensure resident health and safety needs are met while maintaining compliance with state regulations.</li><li>Coordinates with 3rd party providers to ensure continuity of care for residents receiving outside services such as hospice or home health.</li><li>Shares on-call and manager on duty responsibilities/shifts as needed.</li><li>Participates in department quality improvement activities.</li><li>Participates in or leads meetings relevant to resident care issues including care plan meetings with appropriate parties as needed.</li><li>Engages in departmental recruitment activities to maintain adequate staffing levels in the community.</li><li>Other duties and projects as assigned.</li></ul><p><strong id=\"isPasted\" style=\"font-weight: 700; font-size: 18px; font-style: normal; text-align: left; text-indent: 0px;\"><span style=\"font-size: 24px;\">Requirements &amp; Qualifications</span></strong>&nbsp;</p><ul><li id=\"isPasted\">State Nursing License (LVN/LPN or RN)</li><li>Board-approved degree or certificate in nursing, health sciences, or a related field</li><li>Minimum of 3 years relevant experience, clinical leadership experience preferred.</li><li>Experience working with Memory Care residents is preferred.</li><li>Knowledge of RCFE Regulations</li><li>Strong critical thinking and problem solving</li><li>Technically savvy</li><li>Strong customer service skills</li><li>Patience and compassion when dealing with others, especially those who are sick, injured, elderly, or from different backgrounds.</li><li>Valid Driver’s License</li></ul>",
