Responsible for coordination of activities based on identified social barriers to optimal health, including those revolving around transportation, food, housing and education/employment needs. Provides education on and acts as a bridge to the US health system for immigrants and refugees.</p><p><br></p><p><strong>MAJOR DUTIES &amp; RESPONSIBILITIES: </strong>&nbsp;</p><ul><li>Conducts in-depth assessment of patients identified as having social issues through the PRAPARE tool to identify strengths and opportunities for growth;</li><li>In conjunction with patient, develop a plan to address social issues;</li><li>Provides referrals, coaching and mentoring to ensure patients have opportunities to address social needs;</li><li>Follow-up with patients to determine progress and provide coaching/mentoring as required.</li><li>Stay up to date with social service resources;</li><li>Service and respond to customer and patient questions, issues and/or concerns;</li><li>Securely and accurately document all patient information;</li><li>Interpret dictation and respond accordingly;</li><li>Submit Agency forms and documentation in a timely manner. (Improvement activities, response to memorandums, travel reconciliation, supply requests, leave requests, monthly in-service);</li><li>Ensure timely submission of paperwork and reports;</li><li>Communicate with other clinics, specialists, physicians, management, and billing;</li><li>Maintain updated patient information and referral forms;</li><li>Attend on-site/off-site community engagement activities and on-site/off-site clinic events as needed;&nbsp;</li><li>Positively contribute to staff morale and corporate culture;</li><li>Ensure that work assignments and information gathered from day-to-day work is not shared with anyone and protects all patient and agency information.
