    "ExternalDescriptionStr": "<div style=\"font-family: &quot;Segoe UI&quot;; font-size: 14px; font-style: normal; font-weight: 400; line-height: 20px;\"><h3><strong>Patient Care Navigator – Kidney Care</strong></h3><p><strong>Location:</strong> Gateway + rotating support across Nephrology Clinics IU North, IU Fishers, IU Methodist, IU West, and University Hospital<br><strong>Schedule:</strong> Monday – Friday, 8:00 AM – 4:30 PM<br><strong>Work Arrangement:</strong> Hybrid (Must reside in Indiana)</p><hr><h3><strong>Role Overview</strong></h3><p>This position supports the Kidney Care Management program by managing a patient caseload and providing outreach, education, and care coordination for individuals with chronic kidney disease (CKD). The role blends telephonic, electronic, and in-person support to improve patient engagement, outcomes, and access to care.</p><hr><h3><strong>Key Responsibilities</strong></h3><ul><li>Conduct outreach and <strong>cold-call patients</strong> to introduce and enroll them in the program</li><li>Complete <strong>monthly check-in calls</strong> to monitor patient progress and support care plans</li><li>Perform <strong>ED follow-up calls</strong> to ensure continuity of care post-discharge</li><li>Provide patient education and coaching to support self-management and clinical goals</li><li>Assist with scheduling appointments (nephrology, imaging, transplant coordination)</li><li>Identify patient needs and coordinate referrals (e.g., Social Work for SDOH barriers)</li><li>Support patients with:<ul><li>Transportation coordination</li><li>Visit preparation (questions, expectations)</li><li>Access to community and government resources</li></ul></li><li>Document all patient interactions and maintain accurate records</li><li>Collaborate closely with a <strong>multidisciplinary care team</strong></li><li>Participate in program evaluation and process improvement efforts</li></ul><p><i>If filled by a Registered Dietitian (RD), the role will also include providing dietary education to patients.</i></p><hr><h3><strong>Team Environment</strong></h3><ul><li>Optimistic and <strong>growth-oriented</strong> team culture</li><li>Collaborative and flexible environment</li><li>Opportunity to help <strong>build and shape a developing program</strong></li></ul><hr><h3><strong>Qualifications</strong></h3><ul><li>High school diploma or equivalent required; Bachelor’s degree preferred</li><li>One of the following preferred:<ul><li>LPN</li><li>Registered Dietitian (RD)</li><li>Medical Assistant (MA)</li><li>Bachelor’s in Social Work (BSW)</li></ul></li><li>Experience working with patients with <strong>chronic or behavioral health needs</strong></li><li>Experience in a <strong>multidisciplinary care setting</strong></li><li><strong>Experience in kidney care is preferred</strong></li></ul><hr><h3><strong>Skills for Success</strong></h3><ul><li>Self-starter who is comfortable working independently</li><li>Confident in <strong>cold-calling and patient outreach</strong></li><li>Strong communication and relationship-building skills</li><li>Comfortable working with the CKD population</li><li>Organized, adaptable, and mission-driven</li></ul></div><p>&nbsp;</p>",
