    "description": "<h2>Overview</h2>\n<p>Assesses patient/family health problems and resources, takes a leadership role in the development,  implementation and outcomes evaluation of a plan for nursing care provided by the nursing team. Provides  technical nursing care and interventions to designated patient populations. </p>\n<p>This position requires providing service to a geriatric population in a manner that demonstrates an understanding  of the functional/developmental age of the individual served.  </p>\n<p>This position requires the full understanding and active participation in fulfilling the Mission of San Gabriel  Valley Medical Center. It is expected that the employee demonstrate behavior consistent with the Core Values.  The employee shall support San Gabriel Valley Medical Center’s strategic plan and the goals and direction of  the Performance Improvement Plan (PIP). </p>\n<h2>Responsibilities</h2>\n<p>Completes and documents initial assessment/care within required time frames. </p>\n<p>▪ Performs reassessments/care at intervals as required or appropriate to the patient’s specific needs. ▪ Ensures physical assessment/care includes all major body systems. </p>\n<p>▪ Ensures spiritual assessment/care includes actual/expressed needs. </p>\n<p>▪ Incorporates cultural and ethnic factors into assessment/care. </p>\n<p>▪ Correctly differentiates between normal and abnormal findings. </p>\n<p>▪ Makes appropriate referrals for positive high risk screens. </p>\n<p>▪ Identifies educational needs upon admission and thereafter. </p>\n<p>▪ Identifies barriers to learning. </p>\n<p>▪ Provides teaching based on identified needs. </p>\n<p>▪ Evaluates the effectiveness of instruction provided. </p>\n<p>▪ Assessment and teaching incorporates cultural and ethnic factors. </p>\n<p>▪ Assessment and teaching incorporates functional needs. </p>\n<p>▪ Develops and implements the plan of care based on assessment findings. </p>\n<p>▪ Establishes the plan of care on admission. </p>\n<p>▪ Re-evaluates and modifies the plan of care, based on the patient’s response to the interventions. ▪ Communicates the plan of care to members of the nursing team. </p>\n<p>▪ Appropriately coordinates/delegates aspects of the plan of care. </p>\n<p>▪ Involves the patient/family in developing the plan of care. </p>\n<p>▪ Demonstrates sound clinical judgement in decision-making regarding patient care. </p>\n<p>▪ Communicates effectively with physicians and allied health team. </p>\n<p>▪ Coordinates nursing care with other disciplines involved. </p>\n<p>▪ Involves allied health team members, as necessary. </p>\n<p>▪ Actively participates in multidisciplinary care conferences. </p>\n<p>▪ De-escalation of patients </p>\n<p>▪ Adhere to “Patients Rights”  </p>\n<p>▪ Understand and follow LPS Law </p>\n<p>▪ Other duties as assigned. </p>\n<h2>Qualifications</h2>\n<p><strong>E</strong><strong>DUCATION</strong><strong>/T</strong><strong>RAINING</strong><strong>/E</strong><strong>XPERIENCE </strong></p>\n<p>Graduate of a RN School of Nursing </p>\n<p>At Least 6 months of RN experience in Psychiatric Nursing or Long Term Care (Not applicable for New Grad) Excellent written and verbal communication skills </p>\n<p>Bilingual (English/Spanish/Vietnamese/Chinese) preferred </p>\n<p>Computer skills</p>\n<p> </p>\n<p><strong>L</strong><strong>ICENSES</strong><strong>/C</strong><strong>ERTIFICATIONS </strong></p>\n<p>Current California RN License </p>\n<p>Current BLS (AHA) </p>\n<p>Current MAB (AB 508) Training or within 3 months of hire </p>\n<p> </p>",
