The Patient Navigator will focus on patient and community engagement, outreach, care coordination, and advocacy. Ensuring individuals receive timely, culturally responsive and person-centered services. The Patient Navigator will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness.
