    "ExternalDescriptionStr": "<p><span>Park Nicollet is looking to hire a Licensed Independent Clinical Social Worker, (LICSW), to join our Outpatient Hospice team!&nbsp; Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.&nbsp; &nbsp;</span></p>\n<p><span><strong><u>Position Summary:</u></strong></span></p>\n<ol>\n <li><span>Provides supportive services to help meet the patient/family social, emotional, economic, and environment needs in relation to the impact of a chronic, acute, or terminal illness though education, support/counseling, and referrals/coordination of community resources.&nbsp;</span></li>\n <li><span>Completes timely visits to meet patient/family needs and ensure program compliance.&nbsp;</span></li>\n <li><span>Collaborates with the interdisciplinary team to provide whole person care addressing bio-psycho-social needs of patients and family.&nbsp;</span></li>\n <li><span>Provide patients/families with the psychosocial support needed to cope with chronic, acute or terminal illness through education, support/counseling and referrals for community resources.&nbsp;</span></li>\n</ol>\n<p><span><strong><u>Work Schedule:</u></strong> 8am-4:30pm M-F.&nbsp;</span></p>\n<p><span><strong><u>Required Qualifications:</u></strong></span></p>\n<ul>\n <li><span><strong>Education, Experience or Equivalent Combination:</strong></span>\n  <ul>\n   <li><span>Master’s degree in social work</span></li>\n  </ul></li>\n <li><span><strong>Licensure/ Registration/ Certification:</strong></span>\n  <ul>\n   <li><span>Must have Minnesota (MN) Licensed Independent Clinical Social Work (LICSW) license upon start date:&nbsp;</span></li>\n   <li><span>Must have vehicle, driver’s license and current auto insurance.
