Description Job Function Summary: Involves the oversight of Licensed Vocational Nurses (LVN) performing a variety of patient care activities, such as administering injections and medications, monitoring patients' blood pressure, changing bandages, inserting catheters, and assisting with bathing or dressing. LVNs are trained to work in all aspects of health care, and may specialize in certain areas. Work is performed in a variety of medical settings, such as hospitals, clinics, physicians' offices and home care. Custom Scope Ensures the quality of LVN patient care and serves as a role model. Assists staff with escalated issues and reports patients’ status and concerns to management or clinicians as needed. Responsibilities % of time Essential Function (Yes/No) Key Responsibilities (To be completed by Supervisor) 40% Yes Supervises and facilitates the flow of LVN activities based on patient needs 20% Yes Ensures staff are performing within their scope of practice, which includes activities such as checking vital signs, administering oral medications, collecting specimens for lab tests and maintaining a safe, clean environment 10% Yes Mentors staff to communicate with patients regarding their conditions, treatments, and concerns 10% Yes Assists with escalated issues and involves management or clinicians as needed 5% Yes Keeps staff informed of changes in applicable policies, procedures, and external regulations 5% Yes Evaluates subordinate staff performance and involved in decision-making on hiring, salary actions, disciplinary actions, performance ratings, and other human resources matters 5% Yes Evaluates the competencies of staff, according to scope of practice for the LVN position 5% Yes Provides orientation and training to staff, monitors and coaches as needed to assure work is performed in adherence to patient service standards, internal policies, external regulations, and quality / quantity expectations Serves as a resource to staff on instructing patients on chronic diseases and self-care management, and with diagnostic and therapeutic procedures and treatments Participates in the performance of routine audits and licensing and accreditation inspection readiness activities Promotes a climate of effective teamwork, communication, and collaboration Communicates regularly with management on practice activities and on pertinent operational or human resources issues Mentors staff and identifies areas for growth Develops and implements strategies for performance improvement Operates within budgetary parameters in the management and distribution of material and fiscal resources Qualifications Required qualifications: At least 2 years or more years of LVN experience within the last 2 years. Ability to organize, manage multiple priorities, and delegate work functions efficiently. Demonstrated skills to effectively supervise patient care support staff. Strong knowledge of practice operations. In depth knowledge of patient support care clinical standards and care delivery, systems, external regulations, and internal policies Working knowledge of human resources management policies, with the ability to train, monitor, evaluate, and document human resources issues and performance, and to participate in decision-making on human resources matters. Detail oriented, with the ability to apply analytical and critical thinking skills to quickly analyze problems, determine appropriate level of intervention, and apply effective solutions. Solid interpersonal skills for effective collaboration and problem solving with all levels of management and staff Skills in written and verbal communications, with the ability to convey clinical and technical information in a clear and concise manner Knowledge of business software and specialized applications and data management systems used in clinic / center patient care. Preferred qualifications: Previous supervisory experience EPIC/APeX Super User CPT I Certified ( CPT-1 certification current with the State of California) Licenses/Certifications/Education: Licensed Vocational Nursing (LVN) License CPR Certification (BLS) through the American Heart Association High school or GED or equivalent combination of education and experience.
