Through a variety of multidisciplinary services that clients can choose from including medication management, therapy, group therapy, health and wellness support, employment&nbsp; services and transportation support, clients can develop skills, access resources and learn ways to manage their emotional and physical health.&nbsp; This role includes working with Deaf and hard-of-hearing clients, ensuring services are accessible, culturally responsive and inclusive of communication needs.</p>\r\n\r\n<p>ReDiscover serves as the Western Deaf Services Specialized Outpatient Center (SOC) for the Missouri Department of Mental Health&rsquo;s Office of Deaf Services. Staff in this role benefit from ongoing consultation and collaboration with the Office of Deaf Services as well as the Eastern Deaf Services SOC team.</p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><span style=\"display:block;font-size:16px;\"><strong>What You&rsquo;ll Do</strong></span></p>\r\n\r\n<ul>\r\n\t<li>Maintain and interact with a caseload of Deaf and hard-of-hearing clients with diverse characteristics either in-person or over the phone.</li>\r\n\t<li>Facilitate each client&rsquo;s achievement of long range outcomes of symptom management, employment and independent living based upon the needs of the client.</li>\r\n\t<li>Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.</li>\r\n\t<li>Help remove barriers for clients, which may include helping clients participate in community or social service programs and referring clients to educational or working programs.</li>\r\n\t<li>Assist clients with transportation barriers as identified in their treatment plan. &nbsp;This includes transporting clients in the employee&#39;s personal vehicle to various appointments and area resources.</li>\r\n\t<li>Collect and maintain information about clients and document interactions within ReDiscover&rsquo;s designated electronic medical record.</li>\r\n\t<li>Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.<br />\r\n\t&nbsp;</li>\r\n</ul>\r\n\r\n<p><span style=\"display:block;font-size:16px;\"><strong>Requirements of the Position</strong></span></p>\r\n\r\n<ul>\r\n\t<li>4 year degree or 4 years of equivalent experience in lieu of degree.</li>\r\n\t<li>A valid automobile driver&rsquo;s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours which includes transporting clients in personal vehicle.</li>\r\n\t<li>Deaf and hard-of-hearing candidates and other native American Sign Language (ASL) users are strongly encouraged to apply.&nbsp; All qualified ASL-fluent candidates will receive full consideration&nbsp; and may qualify for a pay incentive.</li>\r\n\t<li>Experience working with Deaf clients or in a Deaf community setting preferred.</li>\r\n\t<li>Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.</li>\r\n\t<li>Excellent customer service skills with a recovery focus.</li>\r\n\t<li>Strong written, oral and interpersonal communication skills.</li>\r\n\t<li>Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as the use of email.</li>\r\n</ul>\r\n\r\n<p><br />\r\n<span style=\"display:block;font-size:16px;\"><strong>Grow With Us</strong></span></p>\r\n\r\n<p>We&rsquo;re committed to your professional development and will support your growth as a <strong>Community Support Specialist</strong> through training, collaboration, and meaningful work.</p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><strong>Ready to join a team that&rsquo;s passionate about people?
