Overview Alanté Health is seeking a dedicated Nurse Practitioner to redefine the future of home-based primary care in Arizona. If the rush of clinic appointments leaves you wanting a deeper connection with your patients and the time to truly impact their health outcomes, Alanté Health offers a refreshing alternative. In this hybrid role, the Nurse Practitioner will provide comprehensive patient care through home care and telehealth. We provide Chronic, Principal, Transitional and Wellness Care Management for seniors that improves patient outcomes while significantly reducing hospital readmissions and overall healthcare costs. Our solutions not only saves our patients and their families money, it also solves one of their biggest challenges-transportation. We go to them, whether they are at their own home, in Assisted Living or a Skilled Nursing Facility; we flex to make healthcare easier. Responsibilities Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition. Manage acute and chronic conditions, including medication adjustments. Order and interpret diagnostic tests. Develop, implement, and modify treatment plans as needed. Participate in quality assurance and improvement activities. Provide clinical education and training for patients, families, and facility staff. Adhere to state-specific NP scope of practice requirements. Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions. Travel Requirement This role includes in-person patient visits within approximately a 25-mile radius of the clinician’s hiring home address, or as otherwise assigned based on business needs. Travel time during the workday is considered part of the role’s responsibilities and will follow all applicable wage and hour guidelines. Clinicians are expected to maintain a valid driver’s license, reliable transportation, and the ability to meet scheduling requirements for in-home visits. Geographic Assignment Service areas may be adjusted based on patient volume, care coordination needs, and operational demands. Travel expectations apply to all Nurse Practitioners hired under this updated job description. Existing Nurse Practitioners will maintain their originally assigned geographic expectations as part of a grandfathered arrangement, unless they voluntarily opt into the updated travel model. Qualifications •Master’s degree in Nursing or RN license.•Advanced Practice Nursing license.•NP Board Certification from recognized certifying body.•Credentialed with insurance companies preferred.•Fingerprint clearance.•Unrestricted prescribing and dispensing privileges with DEA licensure required.•Driving is a requirement for this position.
