Profit share available after 90 days.•\n<strong>A Collaborative, Supportive Team:</strong> Work alongside professionals who are passionate about our core values - putting people first, delivering exceptional customer experiences, embracing optimism, and executing best practices.\n<h2>Responsibilities</h2>\n<ul>\n <li>Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.</li>\n <li>Manage acute and chronic conditions, including medication adjustments.</li>\n <li>Order and interpret diagnostic tests.</li>\n <li>Develop, implement, and modify treatment plans as needed.</li>\n <li>Participate in quality assurance and improvement activities.</li>\n <li>Provide clinical education and training for patients, families, and facility staff.</li>\n <li>Adhere to state-specific NP scope of practice requirements.</li>\n <li>Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions.<p> </p><p> </p></li>\n <li><p><strong>Travel Requirement</strong></p>\n  <ul>\n   <li>This role includes in-person patient visits within approximately a 25-mile radius of the clinician’s hiring home address, or as otherwise assigned based on business needs.</li>\n   <li>Travel time during the workday is considered part of the role’s responsibilities and will follow all applicable wage and hour guidelines.</li>\n   <li>Clinicians are expected to maintain a valid driver’s license, reliable transportation, and the ability to meet scheduling requirements for in-home visits.</li>\n  </ul><p> </p></li>\n <li><p><strong>Geographic Assignment</strong></p>\n  <ul>\n   <li>Service areas may be adjusted based on patient volume, care coordination needs, and operational demands.</li>\n   <li>Travel expectations apply to all Nurse Practitioners hired under this updated job description.</li>\n   <li>Existing Nurse Practitioners will maintain their originally assigned geographic expectations as part of a grandfathered arrangement, unless they voluntarily opt into the updated travel model.</li>\n  </ul></li>\n</ul>\n<h2>Qualifications</h2>•Master’s degree in Nursing or RN license.•Advanced Practice Nursing license.•NP Board Certification from recognized certifying body.•Credentialed with insurance companies preferred.•Fingerprint clearance.•Unrestricted prescribing and dispensing privileges with DEA licensure required.•Driving is a requirement for this position.
