    "description_html": "<p>This position is responsible for coordinating services for recovery, employment, education, support groups and housing within agency Reintegration programs with intensive clinical supervision of an OASAS approved Qualified Health Professional (QHP).</p><p>Essential Duties and Responsibilities</p><ul><li>Documents individual progress in accordance with agency policy and procedure.</li><li>Prepares discharge summaries in accordance with agency policy and procedure.</li><li>Provides case management, including enhancement of adult daily living skills. Meets with residents regularly to monitor progress.</li><li>Makes referrals, coordinates services and maintains regular ongoing consultations with other providers, legal agencies, community organizations etc. to ensure smooth transitions and minimizes fragmentation of services</li><li>Helps the resident to identify and link with community agencies, resources and supports.</li><li>Coordinates educational and/or vocational skill development.</li><li>Organizes and maintains case records.</li><li>Assists with the admission process and orientation of new residents as needed.</li><li>Ensures the safety of all residents during assigned hours.</li><li>Responds to all immediate social or housekeeping issues as they arise.</li><li>Provides breathalyzer and toxicology screening as needed.</li><li>Responds to any crises and emergencies.</li><li>Completes data input into the agency database.</li><li>Responds in a professional and responsible manner to incidents involving contagious diseases (i.e., HIV, Hepatitis B, TB etc.).</li><li>Assists with the program’s medication self-administration policies and procedures.</li><li>Assists with transportation as needed.</li><li>Facilitates the resident government’s program guidelines.</li><li>Provides resident education.</li><li>Serves on agency committees as requested.</li><li>Carries out additional duties essential to the above-stated responsibilities as assigned.</li><li>Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners.</li><li>Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.</li><li>Regular attendance at work location.</li><li>Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</li><li>Please note all of the above essential duties and responsibilities will include intensive supervision.</li></ul><p><u>Available Shifts:</u></p><p>Sun-Tues 7a-3p, Wed-Thurs 3p-11p</p><p><br></p><p>Sun 7a-3p, Mon-Wed 3p-11p, Sat 7a-3p</p><p><br></p><p>Sun 8a-4p, Mon 7a-3p, Tues 1p-9p, Wed 7a-3p, Thurs 3p-11p</p><p><br></p><p><br></p><p><br></p><p><br></p>",
