When you join the DHHC team, you’ll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve.</p>\n<h2>Responsibilities</h2>\n<p><strong>Medical Social Workers</strong> at the DHHC are integral members of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to patients, allowing them to receive the medical care required in the comfort of their own home.</p>\n<p> </p>\n<p><strong>The Medical Social Worker will:</strong></p>\n<ul>\n <li>\n  <ul>\n   <li>Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.</li>\n   <li>Identify appropriate community resource referrals to address the client’s and the family’s practical and environmental needs.</li>\n   <li>Act as an advocate for both the client and the family, to navigate the patient through the community system.</li>\n   <li>Participate in the development of the total plan of care with the multidisciplinary team of DHHC clinicians, as appropriate.</li>\n   <li>Develop and maintain contracts with public and private agencies as resources for the patient.<p> </p><p>We would love to meet you and tell you more about this engaging and fulfilling part of our healthcare delivery system.
