You will work to provide skilled Physical Therapy services in accordance with the client’s plan of care to include helping clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise.</p>\n<p>We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care.</p>\n<p><em>What you will do in this role:</em></p>\n<ul>\n<li>Provide initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments</li>\n<li>Establish a treatment plan in consultation with the physician and revise it with the approval of the physician as necessary</li>\n<li>Assist in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy</li>\n<li>Assist the physician in evaluating clients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests</li>\n<li>Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise</li>\n<li>Consult and familiarize staff regarding the established plan</li>\n<li>Teach, supervise, and counsel the family and client in the physical therapy program</li>\n<li>Instruct client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices</li>\n<li>Provide the equipment required to carry out the plan. Arrange for out‐client service for procedures which cannot be given at home</li>\n<li>Record on a timely basis all evaluation data, treatments, and client's response to therapy intervention; Also records quality assurance data</li>\n<li>Prepare an assessment of the client's condition and periodic evaluations and summaries. Supervise Physical Therapy Assistants and paraprofessionals as per agency policy</li>\n<li>Record and report to the physician the client's reaction to treatment and /or any changes in the client's condition</li>\n<li>Communicate with the physician at least every 30 days or whenever changes occur regarding the client’s status and obtain additional orders, if necessary.</li>\n<li>Communicate with other team members (including Home attendants) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client</li>\n<li>Write clinical/progress notes on the date of the visit and incorporate the same in the client's record within 72 hours.</li>\n<li>Participate in case conferences, physical therapy staff meetings, agency activities and in‐service programs as necessary</li>\n</ul>\n<p><span style=\"font-weight: bold;\">Qualifications</span></p>\n<ul>\n<li>Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association</li>\n<li>Must be licensed or registered in Virginia</li>\n<li>Must have at least one year of institutional/home health experience</li>\n<li>Must have current CPR certification</li>\n<li>Working knowledge of Medicare regulations and guidelines is a plus</li>\n<li>You must have a minimum of one year experience as an PT, home health experience is strongly preferred</li>\n<li>Must be proficient using PT equipment</li>\n<li>You must be able to provide two references</li>\n<li>You must be able to pass OIG checks, Virginia State Police background check, and national background checks, and able to verify employment eligibility with necessary documentation</li>\n<li>Reliable transportation, proof of valid car insurance, and a valid driver’s license</li>\n</ul>\n<p><span style=\"font-weight: bold;\">Commonwealth Home Care</span></p>\n<p><em>“The Standard of Care from People Who Care”</em></p>\n<p><em>We are an equal opportunity employer and value diversity at our company.
