With over&nbsp;400 full-time, part-time and on-call employees serving children and families in multiple programs throughout Manhattan, the Bronx, Brooklyn and Harlem, Graham Windham provides an exciting &amp; enriching work experience.</span></p><p>To facilitate and lead the process for engaging families, community members, and service providers using Solution Based Casework practice approach in critical decisions related to safety and risk. To provide assessment, engagement, case management and other support services which help ensure that children have strong families to grow up in and have the preparation and opportunity to succeed in school and life. To ensure responsive action is taken to address families&rsquo; needs, accurate and prompt communication and delivery of services in a respectful and culturally appropriate manner. &nbsp;</p><p><strong><ins>Other Duties &amp; Responsibilities (but not limited to):</ins></strong> &nbsp;</p><ul><li dir=\"auto\">Lead conference participants in the group process and become involved in the discussion needed to ensure the best decision in regards to the safety and well-being of the children.<br>&nbsp;</li><li dir=\"auto\">Direct the group process and structure of the conference.<br>&nbsp;</li><li dir=\"auto\">Through the skills of brainstorming, assessment, negotiation and consensus building, create in partnership with families and other participants, a safety plan that reflects the family&rsquo;s strengths and incorporates community resources and addresses the safety, behavioral, educational, and psychological and permanency needs of children<br>&nbsp;</li><li dir=\"auto\">Facilitates discussion of safety and permanency ensuring that:<br>&nbsp;</li><li dir=\"auto\">No family leaves the FTC table until he/she understands the agency&rsquo;s concerns in relation to safety, risk and permanency.<br>&nbsp;</li><li dir=\"auto\">Every family member leaves the FTC table with a clear understanding of the steps to take to achieve the desired case outcomes<br>&nbsp;</li><li dir=\"auto\">Information shared in the conference is clear and straightforward and that all present have a turn to convey the issues as they view them.<br>&nbsp;<ul><li dir=\"auto\">Effectively manages conflict and strong emotions as they arise by utilizing Conflict Management skills.<br>&nbsp;</li><li dir=\"auto\">Accurately records FTC information and decisions on designated forms and in Connections and provide a copy of the report to all conference participants.<br>&nbsp;</li><li dir=\"auto\">Help the family understand agency&rsquo;s concerns in relation to safety and risk.<br>&nbsp;</li><li dir=\"auto\">Promotes an inclusive, safe and supportive environment where all participants will be assured that they can and will be heard.<br>&nbsp;&nbsp;</li></ul></li><li dir=\"auto\">Provides case interventions and Case Management services.<br>&nbsp;</li><li dir=\"auto\">Makes comprehensive family assessments and work towards family well-being, restoration of functioning or maintenance.<br>&nbsp;</li><li dir=\"auto\">Displays good knowledge of cultural, socio-economic and psychosocial aspects of life.<br>&nbsp;</li><li dir=\"auto\">Monitors safety, well-being and progress against other critical outcomes by conducting regular casework contacts in the home and by documenting the same in progress notes and other case documentation.<br>&nbsp;</li><li dir=\"auto\">Creates, in partnership with the families, service plans for families and children which address safety, behavioral, educational, and psychological and permanency needs.<br>&nbsp;</li><li dir=\"auto\">Collaborates with internal and external service providers and other stakeholders through clear and concise timely, verbal and written communication. &nbsp;</li></ul><p><strong><ins>Qualifications</ins></strong> &nbsp;</p><ul><li dir=\"auto\">B.A. or B.S. in Social Work or related fields</li><li dir=\"auto\">Three or more years of experience as a case planner<span style=\"color: rgb(74,74,74);background-color: rgb(255,255,255);font-size: 14px;font-family: ProximaNova,Arial,san-serif;\">;
