Schedules are communicated in advance to support planning and work-life balance.  </p>\n<p><strong>Pay: $28.62 - $35.74</strong></p>\n<p>Our compensation philosophy aims to provide marketable compensation programs and to compensate employees based on relevant experience and education. Human Resources determines the external and internal equitable salary for each employee. </p>\n<p>The above salary range (or hiring range) represents Stony Brook CPMP’s good faith and reasonable estimate of the range of possible compensation at the time of posting</p>\n<h2>Responsibilities</h2>\n<p><strong>SUMMARY:</strong> The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of the Practice.  This position also performs clerical duties, acts as receptionist, maintains medical records and communicates with all departments.</p>\n<p> </p>\n<p><strong>Job Duties & Essential Functions:</strong></p>\n<ul>\n <li>Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider.</li>\n <li>Triage patient phone calls and troubleshoot medication management issues</li>\n <li>Documents nursing observations, interventions, and patient’s response to treatment in patient records.</li>\n <li>Demonstrated competency in performing venipuncture/administering shots for collection of blood specimens and facilitates the prompt and accurate processing and distribution of specimens collected.</li>\n <li>Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the Registered Nurse (RN).</li>\n <li>Accurately performs and records vital signs, weight and height, point of service blood glucose testing, and promptly reports abnormalities to the medical provider or Registered Nurse (RN) on duty.</li>\n <li>Utilizes appropriate techniques when collecting routine urine specimens and specimens for C/S.</li>\n <li>Appropriately instructs patients on clean catch specimens.</li>\n <li>Utilizes appropriate technique in performing venipuncture to obtain blood specimens in accordance with agency procedure.</li>\n <li>Verbalizes knowledge of and adherence to OSHA standards for blood borne pathogens when handling blood and body fluids.</li>\n <li>Maintains standard precautions in the provision of patient care.</li>\n <li>Maintains and monitors stock inventory in the treatment center and replenishes supplies in exam rooms on a timely basis.</li>\n <li>Prepares and processes requisitions for routine supplies in accordance with agency procedures.</li>\n <li>Monitors the integrity of equipment and promptly reports malfunction, missing or damaged equipment to the Registered Nurse (RN) or Office Manager.</li>\n <li>Demonstrates competency in answering phones, patient registration, making appointments, computer data entry and retrieval of reports.</li>\n <li>Copies, prepares records, and stores and maintains appropriate records.</li>\n <li>Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions.</li>\n <li>Conforms to policies and regulations governing patient rights and confidentiality of information.</li>\n <li>Adheres to protocol for patient flow (1. Reception, 2. Registration, 3. Labs, 4. Escort to Provider).</li>\n <li>Perform all other related duties as assigned by management.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Required Education & Qualifications:</strong></p>\n<ul>\n <li>Graduate of an accredited school of Practical Nursing.</li>\n <li>Currently registered in New York State as a Licensed Practical Nurse (LPN). </li>\n <li>Must have excellent communications skills </li>\n <li>Demonstrated competency in performing venipuncture for collection of blood specimens.</li>\n</ul>\n<p><strong>Preferred Qualifications: </strong></p>\n<ul>\n <li>Three years’ experience in ambulatory care setting required as a LPN</li>\n</ul>\n<p><strong>Physical Demands:</strong></p>\n<p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate with patients, staff and medical providers. The employee must be able to exchange accurate information in these situations. This position is active and requires the employee to constantly move about in the office to assist patients, staff and medical providers.
