How you’ll contribute A Case Manager who excels in this role: Complete departmental orientation as well as initial and annual competencies. Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge. Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines. Schedule and conduct family conferences and caregiver communications following team meetings or as needed. Facilitate weekly care team conferences to review patient progress and adjust treatment plans. Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.
