When you join the Visiting Nurse and Hospice for VT & NH, you’ll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve.</p>\n<h2>Responsibilities</h2>\n<p><strong>Medical Social Workers</strong> at Dartmouth Health Home Care<strong> </strong>are integral members of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to patients, allowing them to receive the medical care required in the comfort of their own home.</p>\n<p> </p>\n<p><strong>The Medical Social Worker will:</strong></p>\n<ul>\n <li>Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.</li>\n <li>Identify appropriate community resource referrals to address the client’s and the family’s practical and environmental needs.</li>\n <li>Act as an advocate for both the client and the family, to navigate the patient through the community system.</li>\n <li>Participate in the development of the total plan of care with the multidisciplinary team of VNH clinicians, as appropriate.</li>\n <li>Develop and maintain contracts with public and private agencies as resources for the patient.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Master of Social Work or a Master’s Degree in a related field required.</li>\n <li>Minimum one year experience in a health-related field required.</li>\n <li>Experience with database software or electronic medical record software required.</li>\n <li>MSW and/or LCSW preferred.</li>\n <li>Hospice experience preferred.</li>\n</ul>",
