Description

Position Summary The Families Together Building Solutions (FTBS) program is a home-based, short-term, strength based, solution-focused family preservation program for children and families referred by the Department of Health and Human Services (DHHS). FTBS assists in reducing child abuse/neglect, provides solution-focused services to assist families with meeting behaviorally specific goals, and to keep families intact. These services decrease the risk of child abuse/neglect and out-of-home placement and help effectively meet the needs of at-risk families. FTBS works alongside families to make progress towards identified goals for a period of 90 days, with the possible extension of 90 additional days. FTBS services enhance household management skills, strengthen communication and conflict resolution skills, increases the use and awareness of community resources, teaches and models positive parenting, and offers a wide range of other services to reduce risks and barriers while building on existing strengths of the families served for positive outcomes. Essentials Duties and Responsibilities Provide in-home services following the Families Together Building Solutions (FTBS) model using a strength-based, solution focused approach Conduct in-home face-to-face visits for an average of 3 hours a week per family Assist families in accessing services to meet concrete needs of families and reduce risks/barriers Maintain flexible availiabilty to families Maintain regular communication with referring workers and other service providers Develop service plans for families in collaboration with referral source and family, maintain documentation including case notes, termination reports, and weekly updates with referring workers within designated contract timeframes Develop safety plans for families in crisis or risk of harm to children Complete necessary follow-ups at three and six months, post case-closure Participate in weekly supervision and bi-weekly team meetings Other Knowledge, Skills, and Abilities Ability to maintain confidential information Ability to have a non-judgmental positive attitude toward families in crisis Ability to communicate effectively Ability to work in partnership with other team members and/or service providers Ability to work with a diverse population Ability to multi-task, organize and meet deadlines Knowledge of brief, solution focused therapy and strength based solution focused intervention strategies Knowledge of local resources Recommended Employment Qualifications Education: A Bachelor’s Degree in human services or related field is required. Experience: A minimum of one year of field experience with children and families with multiple problems and the overall ability to relate to and engage with the families. Certificates, Licenses, Registrations: none required. Supervisory Responsibilities: This position does not have supervisory responsibilities.
