CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor,</em> <em>low income</em> <em>people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community.</em>&nbsp;</p><p><br></p><p><strong>POSITION SUMMARY:</strong>&nbsp;</p><p>CHASI’s Prevention Department focuses on seeking out and coordinating outreach, education and referrals for priority populations to address a broad range of needs to improve overall health and wellbeing and decrease acquisition of HIV/HCV and STIs through comprehensive effective strategies.&nbsp; At the core of the service delivery model, Navigators play a critical role in these efforts by aiming to address the barriers clients face in accessing the services they want and need. Navigators will provide case management and referrals services for clients in the target populations in order to provide comprehensive HIV prevention services.&nbsp;&nbsp;</p><p>This is a full time (40 hours per week) position requires a flexible schedule such that some of the job tasks occasionally, with notice, will be conducted during evening hours and on weekends.&nbsp;</p><p><br></p><p><strong>DUTIES &amp; RESPONSIBILITIES:</strong>&nbsp;</p><ul><li>Ability to navigate all referrals from CHASI Testers, program Coordinator, and other staff of priority population clients.&nbsp;&nbsp;</li><li>Conduct intakes, screenings, and linkage &amp; navigation documentation needed to identify the full complement of needs for enrolled persons from priority populations.&nbsp;</li><li>Educate and counsel persons on a variety of prevention education, strategies, and materials according to client presentation and needs.&nbsp;</li><li>Develop, coordinate, and integrate service plans in collaboration with clients.&nbsp;</li><li>Assist clients with appointment scheduling and provider follow ups; plan logistics, provide reminders, assist with transportation, and accompany clients to appointments as needed&nbsp;</li><li>Perform and maintain effective case management for a caseload of clients, as assigned, from intake to discharge.&nbsp;</li><li>Track and monitor client progress and maintain detailed, accurate, and timely notes.&nbsp;</li><li>Be familiar with internal and external services in order to provide appropriate referrals to clients; build relationships and work collaboratively with partner providers.&nbsp;</li><li>Provide clients with linkage, coordination, and referrals to and follow-up with appropriate ongoing providers, including to PrEP, emergency PEP, iART/HIV care, HCV/STI diagnosis and treatment; ensure referrals are successful.&nbsp;</li><li>Work with clients to ensure connection and compliance with appointments and their agreed upon action plan and address barriers&nbsp;</li><li>Collect and submit data to CHASI’s Data Unit accurate data on persons contacted and engaged in services.&nbsp; Maintain updated and accurate records on the services and activities provided.
