Housing Stability Case Manager - Short Term Family Housing Program Washington, DC | Hybrid | $25.96 - $26.92 | Washington Post Top Workplace Do you have a passion for working with families and individuals in need of healthcare, housing, and community support services? Community of Hope is seeking a full-time Housing Stability Case Manager for our Short-Term Family Housing Program. In this role, you’ll help families secure and maintain stable housing, increase their self-sufficiency through income, benefits, and education, and strengthen overall family stability. Join a mission-driven team dedicated to ending homelessness and making a difference every day! This position is located at The Triumph in Southeast, DC. Our Approach and Values: We celebrate people’s strengths and acknowledge the impact of trauma on people’s lives. We embrace diversity, welcome all voices, and treat everyone with respect and compassion. We lead and advocate for changes to make systems more equitable. We strive for excellence and value integrity in all that we do What You’ll Do Works with adults and children to develop case management plans that are updated at least monthly and signed by clients and case manager, which incorporate goals for both adults and children related to core goal areas: finding stable housing; increasing income; and family stabilization and support. Meets weekly and as needed with families on caseload to develop case plans, review progress, and to follow-up on tasks and services. In collaboration with families and Housing Specialist, assists with housing search and placement process, sometimes accompanying families to view units, advocating for families with landlords, and assisting families with applications and other paperwork. Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, etc., and makes referrals as appropriate, including to COH Therapist and other community resources, as appropriate. Completes required assessments with families. Meets regularly with Youth Specialists to ensure children’s needs are part of the parent’s case management plan. Obtains signed release of information forms and communicates regularly with staff at treatment programs, substance abuse programs, and vocational programs.
