This position provides face-to-face screening, assessment, crisis intervention, and individual/group counseling including addressing co-occurring substance use for individuals on the ACT Team. The purpose of this position is to provide behavioral health care counseling services in a culturally diverse community.&nbsp;</p><p><br></p><p><u><strong>ESSENTIAL JOB FUNCTIONS</strong></u></p><p>&nbsp;&nbsp;</p><ul><li>&nbsp;Provides screening, assessment, treatment planning, individual/group counseling, and crisis intervention for clients on the ACT Team; provides information about community resources, refers clients to the most appropriate resources.</li><li>Gathers information through intake interviews; evaluates nature and degree of mental/emotional distress, substance use/abuse, health issues, and social/interpersonal problems; develops individual service plans with clients.</li><li>&nbsp;Conducts short and long-term therapy and/or counseling sessions with using various psychotherapeutic treatment modalities.
