Each case manager will be assigned specific clients at the time of intake and be required to program and monitor such things as court appearances, doctor appointments, family therapy sessions, and discharge referrals.</p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><br></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><strong>Essential Duties and</strong></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><strong>Responsibilities:</strong></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><strong>&nbsp;</strong></p><div style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><ol style=\"margin-bottom:0in;list-style-type: decimal;margin-left: 0.25in;\"><li style=\"margin:0in;font-size:16px;font-family: initial;\">Maintain a caseload of active clients who require case management in such areas as programming court appearances, doctor appointments, family therapy sessions, and discharge referrals.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Act as liaison between courts, families, and outside agencies.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Provide case management services in accordance with the service plan as appropriate to the program.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Coordinate and meet with the members of the Treatment Team and provide them the appropriate reports prior to meeting.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Completes documentation and reports as necessary.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Provides on-call services as needed.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Attends staff meetings as necessary and participates as a team member.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Upholds total confidentiality with respect to clients in all instances.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Ensure that each client is registered for food stamps. Follow up on the completion of the process, working jointly with administrative Case Manager.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Maintain open communication with family, courts, and others as necessary.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Ensure that authorization for Medicaid clients is in on time.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Monitor the billing data/units for Medicaid.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Maintain client case management logbook in accordance with Program/Contract guidelines.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Attends training and in-services as required.</li><li style=\"margin:0in;font-size:16px;font-family: initial;\">Performs other duties as appropriately and assigned.</li></ol></div><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;margin-left:.5in;'><br></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><br></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;margin-left:1.5in;text-indent:-1.5in;'><strong>Qualifications:&nbsp;</strong>To successfully perform in this position, the employee must be able to execute each duty satisfactorily.
