    "ExternalDescriptionStr": "<p style=\"text-align: center;\"><strong>Easterseals NY-Valhalla is looking for a temporary Social Worker for the summer session</strong></p><div style=\"border-width: 0px; font-style: inherit; font-weight: inherit; margin: 0px; padding: 0px; text-align: center;\"><strong>Summer Session runs 7/6-8/14</strong></div><div style=\"color: rgb(0, 0, 0); font-family: Calibri, Helvetica, sans-serif; font-size: 11pt;\">&nbsp;</div><div style=\"color: rgb(0, 0, 0); font-family: Calibri, Helvetica, sans-serif; font-size: 11pt;\">&nbsp;</div><div style=\"border-width: 0px; font-style: inherit; font-weight: inherit; margin: 0px; padding: 0px;\"><br><strong>What You'll Do</strong></div><p>As a Social Worker you will participate as a member of the admissions team and provide client counseling, complete social service assessments and treatment plan maintenance. &nbsp;You will provide a range of education and planning for clients, families and staff.</p><p><strong>Your Responsibilities Will Include:</strong></p><ul><li>Complete social service assessments (to include providing client information on Advanced Directives) within required timeframe of admission as ordered by referring physician.</li><li>Participate in the development and maintenance of the master problem list and social work treatment plans, and identify appropriate short and long-term social service goals for clients.</li><li>Update the master treatment plan based on the changes to the master problem list and social work treatment plans.</li><li>Participate in developing and carrying out behavior management programs as indicated.</li><li>Provide social work assessment, counseling and referral as defined in the client’s treatment plan.</li><li>Assist client, family and team with discharge and long term planning.</li><li>Participate and provide recommendations in all relevant interdisciplinary team conferences.</li><li>Educate team members about specific psychosocial issues that may be impacting on client's treatment plan.</li><li>Establish rapport with families and caregiver support systems with client consent and facilitate family meetings as needed.</li><li>Support/educate clients to relevant social, economic and mental health concerns to maximize independent living in individual and group settings.</li><li>Facilitate appropriate psychotherapy and psycho-educational groups.</li><li>Carry on a positive working relationship with both inter- and intra-Agency sources.</li><li>Maintain professional ethics and adhere to the NASW code of ethics.</li><li>Maintain documentation as defined in Agency’s Policy Manual.</li></ul><p style=\"margin-bottom: 0in;\"><span><strong>You’re a Great Fit for this Role if You Have:</strong></span></p><ul><li>Bachelor’s Degree in Social Work or related Human Service’s field</li><li>Two (2) years experience providing social work services to the respective community.</li><li>Demonstrate a genuine interest in working with the clients through their rehabilitative and psychosocial treatment process</li><li>Possess ability to function as an integral member of a multi-disciplinary rehabilitation team</li></ul><p>&nbsp;</p><p><span style=\"color: black;\"><strong>Compensation&nbsp;</strong></span></p><ul><li><span style=\"color: black;\">&nbsp;$50-55/hr.&nbsp;</span></li></ul><p>&nbsp;</p><p><strong>Who We Are</strong></p><p>Easterseals is a standard-bearer for creating equity and access for people with disabilities and other barriers to long term self-sufficiency.
