Position Description Northern Pines Mental Health Center is proud to offer competitive wages and an exceptional benefits package! We are seeking a Mental Health Practitioner to join our Safe Harbor team. This is a full-time role working weekends from 7am – 7pm . This position will primarily be located at our Crisis Bed facility. in Brainerd, MN. Essential Functions include: Complete required intake information with individuals, including the Treatment Consent, Authorization for Release of Information, Emergency Contacts, Property Inventory, Orientation to property and program, review policies and procedures, and MN-its verification Inventory medications and complete the Electronic Medication Administration Record (eMAR) when the RN is not on site. As an unlicensed assistive personnel (UAP) follow the nurses' protocol for the administration of all medicines Complete the following assessment, Individual Abuse Prevention Plan (IAPP), Immediate Needs Assessment (INA), Individual Treatment Plan (ITP), and substance use screening and assessment (CAGE-AID) Complete a PHQ-9 and Columbia-Suicide Severity Risk Scale (C-SSRS) with each individual at intake. Complete daily after that if the PHQ-9 score is Become familiar with every individual's treatment plan and goals Update and complete ITP review at least one time weekly or when ITP goals are met Daily, exchange information about individuals served as necessary to effectively care for the individual and to follow and update an individual’s treatment plan and individual abuse prevention plan Show leadership while on duty and maintain authority, safety, and standards of performance and be flexible to other tasks assigned to meet IRTS/RCS programming changing needs Provide crisis intervention and social conflict resolution as necessary to assist individuals who are experiencing mental health symptoms Conduct Integrated Dual Diagnosis Treatment (IDDT), Illness Management, and Recovery (IMR) sessions as scheduled using the developed curriculum Document on each individual daily. Record attendance of all groups in a progress note Collaborate with the Safe Harbor staff on shift and complete the Residential Daily Summary on each individual Coordinate the discharge plan with the individual and necessary support people and develop the discharge summary Complete a discharge procedure checklist along with the audit and close chart checklist. Scan completed chart to records [email protected] and inform of which supports will need a copy of discharge summary for Medical Records to fax The individual and the referent complete the discharge satisfaction survey at discharge request. Follow up with the individual at 30 days to complete a telephone survey Assist with meal preparation and cleanup when needed and encourage individuals to assist as well Transport individuals as needed to appointments Keep the staff office clean and organized to project a professional appearance and keep the information confidential Attend required weekly team meetings the week you are scheduled to work. Record in EHR on the Clinical Supervision and Case Consultation note. If unable to attend, complete the ancillary team meeting note in EHR with a Practitioner who was in attendance by your next shift worked Qualifications include: Must meet Mental Health Practitioner status per the State of Minnesota in at least one of the following ways: Holds a master's or other graduate degree in behavioral sciences or related fields. OR Has at least 2,000 hours (approximately one year working full-time) of work experience in the delivery of mental health services. OR Has completed at least 30 semester hours or 45 quarter hours of coursework in Behavioral Sciences or related fields AND has completed a practicum or internship that required direct interaction with adults or children served, and was focused on behavioral sciences or related fields Valid Driver’s License and ability to operate a motor vehicle Must successfully pass a MN DHS NET Study background check prior to hire Current CPR and First Aid Certification is required. Training will be provided if not currently certified.
