Change Lives with Mental Health Kokua.</p><p>Are you passionate about making a difference in the lives of adults experiencing homelessness and mental illness? Mental Health Kokua (MHK) is seeking a <strong>CIS Homeless Outreach Case Manager</strong> to join our Oahu team. In this pivotal role, you&rsquo;ll deliver outreach and housing transition services, connect clients with essential community resources, and serve as a trusted advocate on their path to stability and recovery.</p><p>As a Case Manager, you&rsquo;ll build relationships with clients in the field, coordinate care with partner agencies, and use your problem-solving skills to help individuals overcome barriers and achieve their goals.</p><p><strong>CIS Homeless Outreach Case Manager Responsibilities:</strong></p><ul><li>Maintain a caseload of up to 15 clients and meet with each client at least four times per month.</li><li>Provide field-based outreach to engage adults with serious mental illness who are not connected to care.</li><li>Use motivational interviewing, incentives, and relationship-building to establish trust and promote participation in services.</li><li>Assess needs, coordinate referrals for psychiatric, medical, and housing services, and assist with obtaining identification and health plan enrollment.</li><li>Complete and document clinical assessments, service plans, progress notes, and data reporting in accordance with agency and funding requirements.</li><li>Participate in case conferencing and collaborate with the Continuum of Care, Ohana Health Plan, AMHD, and other providers to coordinate housing placement.</li><li>Enter data in the Homeless Management Information System (HMIS) and complete monthly reporting.</li><li>Confirm insurance coverage and payer authorizations regularly.</li><li>Maintain accurate, confidential client records and adhere to all state and federal regulations.</li><li>Participate in supervision, training, and team meetings to ensure high-quality, person-centered services.</li><li>Perform other duties as assigned by management.</li></ul><p><strong>CIS Homeless Outreach Case Manager Requirements:</strong></p><ul><li>Minimum qualifications include a high school diploma and three years of work experience in human services.</li><li>Preferred qualifications include a bachelor&rsquo;s degree with coursework in psychology or social work and experience supporting individuals with co-occurring mental health and substance use challenges.</li><li>Candidates must demonstrate strong interpersonal, communication, and problem-solving abilities; be tolerant and effective in crisis situations; and have the flexibility to work non-traditional hours as needed.</li><li>Required certifications and credentials include current CPR, First Aid, and TB Clearance; a valid Hawaii driver&rsquo;s license, vehicle insurance, safety inspection, and a clean driving abstract; and the ability to pass a criminal background check.</li><li>Must be able to lift up to 25 lbs and travel throughout Oahu to engage clients in the field.</li></ul><p><strong>Benefits of Joining MHK:</strong></p><ul><li>Medical &amp; Vision Insurance (UHA or Kaiser)</li><li>Dental Insurance (HDS)</li><li>Supplemental &amp; Pet Insurance</li><li>403B Retirement Plan</li><li>Identity Theft &amp; Legal Protection</li><li>Paid Leave &amp; Holiday Pay</li></ul><p><strong>Why Choose MHK?</strong></p><p>At Mental Health Kokua, we believe in &ldquo;Opportunities to Begin Again.&rdquo; As a CIS Homeless Outreach Case Manager, you&rsquo;ll be part of a team committed to helping individuals regain stability, dignity, and hope.
