Facilitate individual growth in the areas of personal responsibility, social skills, and community living skills. We are a strong-knit community that values the ideas and contributions from our staff. </p>\n<p> </p>\n<p><strong>Are you ready to make a difference?</strong></p>\n<h2>Minimum Education Required</h2>High School Diploma/GED\n<h2>Responsibilities</h2>\n<ul>\n <li>Provide ongoing support, guidance and role modeling for individuals.</li>\n <li>Provide individual, informal counseling with individuals to discuss concerns and develop goals as needed.</li>\n <li>Assist residents with home management and community activities, self-advocacy, decision-making and empowerment, following goals appropriately.</li>\n <li>Assist residents with all ADLs, including showering, dressing, grooming, toileting, mobility, feeding, taking medications, and Range of Motion (ROM) exercises, both in the home and in the community as needed.</li>\n <li>Assist with weighing program participants, take vital signs, gathers and records other data as needed.</li>\n <li>Assist with teaching appropriate social and independent living skills and being a positive role model</li>\n <li>Encourage program participants to perform tasks as independently as possible, assisting them to participate in all activities as appropriate.</li>\n <li>Attend and actively participate in resident, family, team and staff meetings and supervisions.</li>\n <li>Assist program participants with taking medications in accordance with MAP regulations.</li>\n <li>Accurately document observations and shift notes using computer database. </li>\n <li>Alert Program Management in the event of a program participant’s change in status or behavior.</li>\n <li>Complete all documentation and Incident Reports within program timelines.</li>\n <li>Ensure a clean, safe and home-like environment for clients.</li>\n <li>Remain alert at all times throughout scheduled work shift.</li>\n <li>Participate in core training and maintain current certification of required trainings (CPR/First Aid, crisis management, MAP) and maintain certification of required trainings</li>\n <li>Provide crisis intervention and access emergency services as needed.</li>\n <li>Perform physical intervention in the event of a client crisis.</li>\n <li>Transport individuals using Advocates vehicles, including wheelchair lift-equipped vans as well as employee’s personal vehicle as needed.</li>\n</ul>\n<h2>Qualifications</h2>\n<p> </p>\n<ul>\n <li>High School Diploma or equivalent degree required. Experience in providing direct care preferred.  </li>\n <li>Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.</li>\n <li>Must be able to perform each essential duty satisfactorily.</li>\n <li>Must possess strong computer skills</li>\n <li>High energy level and ability to function in a team atmosphere and independently.</li>\n <li>Ability to understand, speak, read and write fluent English or the language of the program assigned.</li>\n <li>Ability to communicate effectively both verbally and in writing and ability to use good judgment.</li>\n <li>Sensitivity to the needs of the individuals we support </li>\n <li>Must have a valid driver’s license and access to an operational and insured vehicle in addition to willingness to use it to transport residents. </li>\n</ul>\n<p> </p>\n<p><em>Advocates is committed to cultivating a diverse and welcoming community where everyone feels respected and valued.
