Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive.  We take a comprehensive approach by offering more than 180 integrated programs in:  Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach more than 72,000 individuals and families each year at our 100 locations, including 10,000 youth.  CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families live in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today. CAMBA is partnering with NewYork-Presbyterian Brooklyn Methodist Hospital to have Community Health Workers (CHWs) perform health-related educational services in connection with the hospital’s Adult Community Health Navigation Initiative. The CHWs will receive referrals from the hospital to consent and enroll individuals into the program and then work with them for approximately 6 months. The CHWs will accompany them to appointments; make referrals to appropriate services; provide health education; help to develop educational workshops and health fairs; and perform other duties as assigned. CHW time and work is split between being on-site at NewYork-Presbyterian Brooklyn Methodist Hospital and CAMBA, with NewYork-Prebyterian assigned work and tasks taking priority. Therefore, a split management model is utilized, in which the CHW reports to a Program Manager at CAMBA and a Program Manager at NewYork-Prebyterian who consult with one another regularly. Position: Community Health Worker Reports To: Program Manager of CAMBA Care Connections Location: 2244 Church Ave. What Does a Community Health Worker Do: Maintain professional relationships with clients and client confidentiality.  Practice Universal Precautions/Standard Protocol & Procedures.  Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.  Participates in outreach, planning and execution of special events, including workshops, health fairs, jointly agreed upon community events, the annual Community Health events such as the family graduation, and staff meetings & events.  Attends local project case management meetings and attends practice based interdisciplinary team meetings.  Attends patient medical visits and accompanies patients at social service visits, as needed.  Collaborates with other NYP and CAMBA team members such as patient navigators, community navigators, social workers, care managers, and care coordinators to improve patient outcomes.  Visits the homes of program participants to provide in person education and support, along with other activities, including home environmental assessment, as determined by specific program.  Conduct telephone and online directory searches to secure up-to-date contact information for eligible individuals.  Educate eligible individuals about the health benefits of the program (i.e., email and/or mail program & enrollment information, make phone calls to individuals, their Health Home providers and family members).  Schedule intake appointments between the eligible individuals and the care management team as needed.  Conduct initial needs assessments of clients and clients' families.  Work with clients to break through barriers to their goals and to assist them in advocating for themselves and in moving toward self-sufficiency.  Assist clients in creating and maintaining client files, including completing applications for benefits and entitlements (e.g. Medicaid, etc.), and process applications on clients' behalf.   Enter and monitor client’s data, progress, outcomes towards their service goals and document all interactions with clients via progress notes, and relevant automated database.  Follow all HIPAA privacy rules as they relate to personal health information and documentation related to clients.  Provide all required information for weekly/monthly/quarterly/annual reports (i.e. client services reports, documentation ticker/ tracking reports, client contact reports, vehicle incident reports).  As needed assist with telehealth visits, medication reconciliation, social/peer support events, client follow-up, and support for CAMBA-eligible individuals.  Provide education and support to patients, including how to check and track their blood pressure, make healthy lifestyle changes (diet, physical activity), and manage medications. Other duties as assigned.  What are the Qualifications for the Position: Education, Licenses and/or Certifications Required: Sufficient Education: to comprehend basic reading, writing, and math to execute required functions of the role.  High school diploma or G.E.D. preferred.  Minimum Experiences and Skills Required: Two (2) years of experience as a Community Health Worker or working within the Health Care field providing services such as patient outreach, recruitment and enrollment; patient education, and/or care management.  Knowledge about, understanding of, and ability to work closely with, persons with chronic illness and related issues.  Knowledge of community resources and sensitivity toward persons with chronic illnesses.  Some evenings and/or weekends may be required.   Ability to travel within the five boroughs of New York City.  Bi-lingual in English/Cantonese strongly preferred.  Tech Savy   Proficiency in Microsoft Office (E.g. Word, Outlook, Excel)  Compensation : - $50,000 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization. Therefore, a split management model is utilized, in which the CHW reports to a Program Manager at CAMBA and a Program Manager at NewYork-Prebyterian who consult with one another regularly.   </p><p></p><p><strong>Position: Community Health Worker </strong></p><p><strong>Reports To: Program Manager of CAMBA Care Connections </strong></p><p><strong>Location: 2244 Church Ave.</strong></p><p><b>What Does a Community Health Worker Do:</b> </p><ul><li>Maintain professional relationships with clients and client confidentiality. </li><li>Practice Universal Precautions/Standard Protocol & Procedures. </li><li>Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information. </li><li>Participates in outreach, planning and execution of special events, including workshops, health fairs, jointly agreed upon community events, the annual Community Health events such as the family graduation, and staff meetings & events. </li><li>Attends local project case management meetings and attends practice based interdisciplinary team meetings. </li><li>Attends patient medical visits and accompanies patients at social service visits, as needed. </li><li>Collaborates with other NYP and CAMBA team members such as patient navigators, community navigators, social workers, care managers, and care coordinators to improve patient outcomes. </li><li>Visits the homes of program participants to provide in person education and support, along with other activities, including home environmental assessment, as determined by specific program. </li><li>Conduct telephone and online directory searches to secure up-to-date contact information for eligible individuals. </li><li>Educate eligible individuals about the health benefits of the program (i.e., email and/or mail program & enrollment information, make phone calls to individuals, their Health Home providers and family members). </li><li>Schedule intake appointments between the eligible individuals and the care management team as needed. </li><li>Conduct initial needs assessments of clients and clients' families. </li><li>Work with clients to break through barriers to their goals and to assist them in advocating for themselves and in moving toward self-sufficiency. </li><li>Assist clients in creating and maintaining client files, including completing applications for benefits and entitlements (e.g. Medicaid, etc.), and process applications on clients' behalf.  </li><li>Enter and monitor client’s data, progress, outcomes towards their service goals and document all interactions with clients via progress notes, and relevant automated database. </li><li>Follow all HIPAA privacy rules as they relate to personal health information and documentation related to clients. </li><li>Provide all required information for weekly/monthly/quarterly/annual reports (i.e. client services reports, documentation ticker/ tracking reports, client contact reports, vehicle incident reports). </li><li>As needed assist with telehealth visits, medication reconciliation, social/peer support events, client follow-up, and support for CAMBA-eligible individuals. </li><li>Provide education and support to patients, including how to check and track their blood pressure, make healthy lifestyle changes (diet, physical activity), and manage medications.</li><li>Other duties as assigned. </li></ul><p><b>What are the Qualifications for the Position:</b> </p><p><b>Education, Licenses and/or Certifications Required:</b> </p><ul><li>Sufficient Education: to comprehend basic reading, writing, and math to execute required functions of the role.  High school diploma or G.E.D. preferred. </li></ul><p><b>Minimum Experiences and Skills Required: </b> </p><ul><li>Two (2) years of experience as a Community Health Worker or working within the Health Care field providing services such as patient outreach, recruitment and enrollment; patient education, and/or care management. </li><li>Knowledge about, understanding of, and ability to work closely with, persons with chronic illness and related issues. </li><li>Knowledge of community resources and sensitivity toward persons with chronic illnesses. </li><li>Some evenings and/or weekends may be required.  </li><li>Ability to travel within the five boroughs of New York City. </li><li>Bi-lingual in English/Cantonese strongly preferred. </li><li>Tech Savy  </li><li>Proficiency in Microsoft Office (E.g. Word, Outlook, Excel) </li></ul><p><strong>Compensation</strong>: - $50,000 annually<br><em>When salary ranges are listed, the range would represent the low and high end for the applicable position & program.
